Cancer affects individuals in all human dimensions. Cancer patients are more susceptible to spiritual distress. Several studies have addressed spiritual distress using quantitative designs; however, a qualitative approach to the experience of spiritual distress could provide a deeper understanding of the phenomenon. This study aims to synthesis the experience of spiritual distress as lived by cancer patients in qualitative primary studies. This is a literature review based on electronic databases search. A total of 4075 citations was identified and 23 studies were included. The most frequent qualitative research method was phenomenology (n = 15), and interviews were the main data collection method (n = 20). Two major themes have been identified related to the experience of spiritual distress: suffering and coping. Spiritual distress is an intimate, deep and suffering experience in life, which requires coping strategies and involves spiritual values and beliefs. Healthcare providers should be aware of this experience and recognize spiritual distress in cancer patients, as it is critical in providing holistic nursing care.

Introducción: En la actualidad, un gran número de personas conviven con cáncer en cualquier etapa de la enfermedad, con todas las repercusiones físicas, emocionales y socio-culturales que esta patología implica. Este estudio explora las experiencias de los pacientes con diagnóstico de cáncer, y la vivencia de los cuidados de enfermería, de tal manera que permita comprender la integralidad del proceso de la enfermedad. Materiales y Métodos: Meta-estudio cualitativo de tipo de análisis principalmente de meta-datos y meta-síntesis, con 12 artículos seleccionados previa revisión exhaustiva de criterios de rigor metodológico en credibilidad, auditabilidad y transferibilidad. Las bases de datos empleadas fueron ISI Web of Knowledge, Pudmed y Scielo. Resultados: Vivir con cáncer significa 1) Capacidad de adaptación, 2) Redescubrir emociones, 3) Ser consecuencia del tratamiento, y 4) Disociar mente y cuerpo. Discusión: La vivencia del cáncer es un fenómeno social y humano complejo, que desafía el derecho individual de salud de las personas en el ámbito sanitario, marcada por la alta mortalidad e incapacidad asociada y establecida por determinantes sociales de la salud. Conclusiones: La vivencia del paciente con cáncer, sin duda es una compleja dinámica que siempre se verá beneficiada con la exploración cualitativa de su existencia, donde los análisis de los hallazgos proporcionados en su conjunto contribuyen sin duda a transcender y comprender mejor el fenómeno del cáncer. Con ello presentando desafíos importantes para la enfermería en materia de perfeccionar sus competencias para entregar un cuidado humano, comprensivo y particular. It is not uncommon for patients with advanced disease to express a wish to hasten death (WTHD). Qualitative studies of the WTHD have found that such a wish may have different meanings, none of which can be understood outside of the patient's personal and sociocultural background, or which necessarily imply taking concrete steps to ending one's life. The starting point for the present study was a previous systematic review of qualitative studies of the WTHD in advanced patients. Here we analyse in greater detail the statements made by patients included in that review in order to examine their moral understandings and representations of illness, the dying process and death. We identify and discuss four classes of assumptions: (1) assumptions related to patients' moral understandings in terms of dignity, autonomy and authenticity; (2) assumptions related to social interactions; (3) assumptions related to the value of life; and (4) assumptions related to medicalisation as an overarching context within which the WTHD is expressed. Our analysis shows how a philosophical perspective can add to an understanding of the WTHD by taking into account cultural and anthropological aspects of the phenomenon. We conclude that the knowledge gained through exploring patients' experience and moral understandings in the end-of-life context may serve as the basis for care plans and interventions that can help them experience their final days as a meaningful period of life, restoring some sense of personal dignity in those patients who feel this has been lost.
