MRI and 31 P magnetic resonance spectroscopy hardware for axillary lymph node investigation at 7T

Abstract: The first (31) P MR spectrum from an in vivo lymph node indicates that the presented design may be sufficiently sensitive to detect metabolic response to neoadjuvant therapy. Multinuclei MRS of the lymph nodes at 7T is possible through combining lightweight antenna elements with dual-tuned transceiver/receive-only coils.

“…Polarization transfer techniques transfer the polarization of excited 1 H spins through J-coupling to the 31 P spins, increasing the signal-to-noise ratio and eliminating resonances without 1 H- 31 P coupling, thereby flattening the baseline and significantly increasing the signal-to-noise ratio of PE, PC, GPE and GPC at 1.5T [107], 3T [100] and 7T [106]. Using specialized hardware to adapt high-field 31 P MRS at 7T, it was possible to detect 31 P MR spectra from a small 3-mm diameter lymph node in the axilla [108]. These recent advances in 31 P MRS coil and pulse sequence design will enable the detection of individual PE, PC, GPE and GPC in cancers in the near future.…”

“…The tumor size that is possible to investigate with 7 T depends on the location of the tumor. For example, a sub mL volume (<0.1 mL) of signal‐generating tissue with <2 mmol concentrations of PMEs has been detected above the noise floor in healthy axilla lymph nodes . In the breast, a < 2 mL volume tumor was monitored for treatment tracking using 31 P at 7 T .…”

“…Although the 23 Na solenoids provided good coverage, 1 H coverage was confined primarily to the breast and degraded toward the axillary and chest wall regions. This is a common weakness in 7T 1 H breast MRI, which has been addressed with dedicated hardware . In the case of our design, space constraints for the multiple bilateral coil layers may have accentuated its compressed local field pattern.…”
