MRI of the penis

Abstract: ABSTRACT. MRI of the penis is an expensive test that is not always superior to clinical examination or ultrasound. However, it shows many of the important structures, and in particular the combination of tumescence from intracavernosal alprostadil, and highresolution T 2 sequences show the glans, corpora and the tunica albuginea well. In this paper we summarise the radiological anatomy and discuss the indications for MRI. For penile cancer, it may be useful in cases where the local stage is not apparent clinic… Show more

“…Intracavernosal alprostadil 4.1 Efficacy and clinical evidence ICI of alprostadil mimics the physiologic mechanisms of cavernosal smooth muscle relaxation [20] and as such is used as a second-line treatment for men with ED who have failed to respond to a PDE5 inhibitor or who were unable to take such medication. It is also used to produce an artificial erection during color Doppler assessment of the penile vasculature, during the assessment of men with Peyronie's disease (when the degree of deformity can be accurately assessed in the clinic) and occasionally to aid MRI staging of the penile cancer [21,22].…”

“…MRI was used for the 10 most recent patients in our series and was found to be a valuable diagnostic tool (Figure 2(b)). MRI is considered superior to other imaging methods due to its excellent ability to visualize all penile structures [3], but excluding a penile fracture with a negative MRI is still uncertain [10]. MRI may also exclude important differential diagnoses like rupture of the circumflex or dorsal vein of the penis [3].…”

“…However, MRI and cavernosography are mostly recommended to depict discontinuity of the tunica albuginea and diagnose the penile fracture. [6][7][8] The precise diagnosis can be confirmed only after surgical exploration. 5,9 The few reported cases of rupture of the superficial dorsal vein of the penis sometimes imitate a penile fracture; this rupture is thus sometimes called a "false fracture."…”
