“…Its clinical significance relates to the relative position of the Onodi cell to the optic nerve. The pathogenesis is thought to be that of direct mechanical compression from the enlarging mucocele causing a subsequent circulatory disturbance with ischaemia resulting in an optic neuropathy [2, 3]. This paper is of optic neuropathy secondary to an Onodi cell mucocele in a young Chinese patient who had complete recovery of visual function following surgical decompression despite a relatively long time interval between the onset of symptoms and surgery.…”

“…However, these findings are nonspecific. Differentiation from a underlying tumor is aided by the addition of MRI examination preoperatively [5]. While it is likely that initially a mucocele would demonstrate a fluid signal in conventional MRI sequences, over time water is resorbed from the collection and the proteinaceous content increases resulting in shortening of T1 relaxation time and characteristic T1 hyperintensity on precontrast sequences [15].…”

“…The discrepancy may be due in part to the fact that older studies using thick section axial CT without isotropic resolution were less sensitive for detecting small air cells which are often flat in configuration within the plane of axial section [4]. Identification of an Onodi cell is of importance prior to sinonasal surgery due to the close anatomic relationship between this posterior air cell and the optic nerve ( [5]; Fig. 1).…”

“…In Japan, the pathogenesis of PSM is classified into idiopathic, postoperative, and posttraumatic cysts [1]. Postoperative and posttraumatic PSMs were identified when patients had clearly undergone sinus surgery or suffered a traumatic injury, respectively.…”
