Mucocele of the surgically isolated esophagus

Abstract: A large esophageal mucocele causing chest pain developed in a 26-year-old woman who had undergone esophageal bypass surgery and gastric interposition because of involvement by scleroderma. Computed tomographic (CT) scans showed the mucocele as an elongated mediastinal mass which spontaneously reduced in size during conservative management. The clinical and radiological features of this unusual postsurgical complication are herein reviewed.

“…10 We also found one case of a large mucocele of the esophagus in a 26-year-old woman who had undergone esophageal bypass surgery and gastric interposition because of involvement by scleroderma. 11 Although the hypothesis that the abnormal mucin deposition in the cutaneous tissues of some SSc patients may have played a role in the development of mucinous cystadenoma in our patient, we thought that this lesion occurred coincidentally in this case. The average age at diagnosis is 50 years and, although early reports suggested that AM occurred more frequently in women than men, this predominance has been challenged in more recent studies.…”

“…Patients reportedly present with non-specific symptoms, including respiratory distress [3,7,9,15], chest [12,15] or abdominal pain [3,9,10], nausea and vomiting [3,9,10], fever [9,14,15], and weight loss [10,11,14]. Others have also presented with recurrent pneumonia [17], sepsis [3], esophago-cutaneous fistulas [9,11,13], and episodic mucoid sputum production secondary to a TE fistula [5].…”

“…Our cases are the only documented cases to have had airway examination in the course of their management. CT or MR imaging of the chest is diagnostic, revealing a fluid-filled dilatation in the mediastinum, and delineating the extent of the mucocele [3][4][5]7,[9][10][11][12]. CT-guided needle aspiration may aid in evaluation of infection [14,15].…”

“…Elders or congregations'), they emphasised that the Spanish had oppressed papists too, and suggested that the tyrants were only using religion as a 'veil' or 'pretense' to hide their lust for worldly domination. 41 Twelve years later, another Reformed manifesto Political Education (1582) reiterated the contention that they were defending the 'fatherland' against the threat of 'eternal slavery'. And they argued eloquently that, since faith was a gift of God, it could not (and should not) be forced.…”
