Mucociliary Respiratory Epithelium Integrity in Molecular Defense and Susceptibility to Pulmonary Viral Infections

Abstract: Mucociliary defense, mediated by the ciliated and goblet cells, is fundamental to respiratory fitness. The concerted action of ciliary movement on the respiratory epithelial surface and the pathogen entrapment function of mucus help to maintain healthy airways. Consequently, genetic or acquired defects in lung defense elicit respiratory diseases and secondary microbial infections that inflict damage on pulmonary function and may even be fatal. Individuals living with chronic and acute respiratory diseases are … Show more

“…SARS-CoV-2 infection is mediated by angiotensin-converting enzyme 2 (ACE2)–containing respiratory epithelial cells, including in the airways and lungs. Infection subsequently leads to transient loss of ciliary motility from the trachea and inhibition of mucociliary clearance, an innate defense against airway pathogens [ 24 ]. Cellular entry and viral replication result in activation of the inflammasome and ignition of the inflammatory cascade.…”

“…Targeted by the bacteria, viruses, or fungi, the impaired cilia disrupted the mucociliary clearance and worsened the infection [ 32 ]. At the COVID-19 infection onset, ciliated cells were the major target [ 33 ], resulting in the cilia shrinking, cilia loss, and ciliary beating reduction [ 34 , 35 ]. In our study, the RC decoction might improve the cilia movement and assembly, promoting the pathogens clearance and alleviating the lung injury.…”

“…The patient believed his chronic allergies may have predisposed him to frequent upper respiratory infections in the few years leading to his hospitalization with pericarditis. Inflammation of the respiratory epithelia impedes mucociliary function and increases the risk of infection [ 28 ]. There have been reports in the literature linking pericarditis to antecedent upper respiratory tract infections [ 29 - 31 ].…”
