A variety of diarrheagenic E coli (DEC) are responsible for causing diarrhea in children under five years age especially in developing courtiers. The aim of this study was to isolate and bacteriological characterizing of DEC from diarrheic children's stool and using molecular identification of DEC pathotypes for better discrimination and investigate their antibiotic resistance patterns. Total of 200 fresh stools specimens were collected from children with diarrhea in Salah Aldin Hospital in Tikrit city, Iraq. The samples were cultured on selective media such as (MacConkey and EMB). Colonies were identified through biochemical reaction and api 20E system and then antibiotic susceptibility profiles were determined. E. coli were isolated and characterized from 200 diarrheagenic stool positive samples, collected from hospitalized children less than 5 years old. A total of 75(40.5%) samples were yielded positive for growth of E. coli Out of these, 46 genotypically-identified DEC were then subjected to multiplex PCR assay targeting certain virulence factors ( SHIG, bfpA, eae, LT, ST, EA, vt1, vt2, daaE, uidA ) for discrimination of pathotyes. 21/46(45.7%) EPEC with 18 atypical and 3 typical, 11/46(24%) ETEC, 3/46 /(6.5%) EAEC, 2/46 (4.3%) for EIEC was detected, 2/46(4.3%) EHEC was also detected, while no DAEC was detected, Also mix pathotypes were detected, more than one pathotype was observed in a number of samples 7/46(15.2%), 5 (aEPEC + ETEC), 2 (aEPEC + EAEC). Moreover, all pathotypes expressed high resistant to Cefotaxime, ampicillin, Piperacillin, Azitromycin, Amikacin. while little resistance to Imipenem, Meropenem, Ciprofloxacin, Norfloxacin was observed. The study concludes EPEC is the dominant pathotype between DEC pathotypes in our society that causes diarrhea in children, and emphasizes the importance of using mPCR assay for best discrimination.

Background and Aim: Camels are important livestock in Egypt on cultural and economic bases, but studies of etiological agents of camelid diseases are limited. The enteropathogen Escherichia coli is a cause of broad spectrum gastrointestinal infections among humans and animals, especially in developing countries. Severe infections can lead to death. The current study aimed to identify pathogenic E. coli strains that cause diarrhea in camel calves and characterize their virulence and drug resistance at a molecular level. Materials and Methods: Seventy fecal samples were collected from diarrheic neonatal camel calves in Giza Governorate during 2018-2019. Samples were cultured on a selective medium for E. coli, and positive colonies were confirmed biochemically, serotyped, and tested for antibiotic susceptibility. E. coli isolates were further confirmed through detection of the housekeeping gene, yaiO, and examined for the presence of virulence genes; traT and fimH and for genes responsible for antibiotic resistance, ampC, aadB, and mphA. The isolates in the important isolated serotype, E. coli O26, were examined for toxigenic genes and sequenced. Results: The bacteriological and biochemical examination identified 12 E. coli isolates from 70 fecal samples (17.1%). Serotyping of these isolates showed four types: O26, four isolates, 33.3%; O103, O111, three isolates each, 25%; and O45, two isolates, 16.7%. The isolates showed resistance to vancomycin (75%) and ampicillin (66.6%), but were highly susceptible to ciprofloxacin, norfloxacin, and tetracycline (100%). The structural gene, yaiO (115 bp), was amplified from all 12 E. coli isolates and traT and fimH genes were amplified from 10 and 8 isolates, respectively. Antibiotic resistance genes, ampC, mphA, and aadB, were harbored in 9 (75%), 8 (66.6%), and 5 (41.7%), respectively. Seven isolates (58.3%) were MDR. Real-time-polymerase chain reaction of the O26 isolates identified one isolate harboring vt1, two with vt2, and one isolate with neither gene. Sequencing of the isolates revealed similarities to E. coli O157 strains. Conclusion: Camels and other livestock suffer various diseases, including diarrhea often caused by microbial pathogens. Enteropathogenic E. coli serotypes were isolated from diarrheic neonatal camel calves. These isolates exhibited virulence and multiple drug resistance genes.

Objectives To investigate the epidemiological features of diarrheagenic Escherichia coli (DEC) in patients with acute diarrhea in China.Methods An active sentinel surveillance was performed in all-age patients with acute diarrhea in China, 2009-2018. DEC was isolated and identified by serological assay and PCR from stool samples.Results DEC was determined in 6.68% (6,119/91,651) of the patients, with higher positive rates among females than among males (6.97% vs. 6.46%) and among 18-59 years patients (7.88%) than among other age groups. Five pathotypes were identified, the most prevalent was enteroaggregative E. coli (EAEC), followed by enteropathogenic E. coli (EPEC), and enterotoxigenic E. coli (ETEC). Pediatric patients < 5 years had higher positive rate of EAEC (2.07%), followed by EPEC (1.81%), and enterohemorrhagic E. coli (EHEC) (0.31%), while the 18-59 years patients had higher infection of ETEC (2.36%). ETEC and EPEC were more frequently identified in urban than rural areas, with age and gender adjusted positive rate of 1.68% vs. 1.14% respectively, and 1.77% vs. 1.55%, while EIEC and EHEC were more frequently identified in rural areas.Conclusions These findings highlight the epidemiology features of DEC and underscores the need for conducting DEC surveillance.
