Compton cameras can offer advantages over gamma cameras for some applications, since they are well suited for multitracer imaging and for imaging high-energy radiotracers, such as those employed in radionuclide therapy. While in conventional clinical settings state-of-the-art Compton cameras cannot compete with well-established methods such as PET and SPECT, there are specific scenarios in which they can constitute an advantageous alternative. The combination of PET and Compton imaging can benefit from the improved resolution and sensitivity of current PET technology and, at the same time, overcome PET limitations in the use of multiple radiotracers. Such a system can provide simultaneous assessment of different radiotracers under identical conditions and reduce errors associated with physical factors that can change between acquisitions. Advances are being made both in instrumentation developments combining PET and Compton cameras for multimodal or three-gamma imaging systems, and in image reconstruction, addressing the challenges imposed by the combination of the two modalities or the new techniques. This review article summarizes the advances made in Compton cameras for medical imaging and their combination with PET.

Various studies that have investigated the detection of gamma coincidence events have revealed that design factors and image reconstruction approaches dictate the spatial resolution, coincidence efficiency, and levels of statistical noise of the detection system. In the case of imaging, cascade gamma-ray coincidence (CGC) imagers coupled with collimated detectors offer promising values for both spatial resolution and coincidence efficiency. However, to date, no CGC imager with single or multiple collimated detectors has reported a performance level beyond 6.7 mm spatial resolution (FWHM) and 6.0 ×1ncidence efficiency. Given the recent developments and the current interests in high resolution and localization of an individual decaying nucleus, there is a need for CGC imagers with higher performance in terms of spatial resolution and efficiency. Therefore, deploying a CGC imager coupled with multiple collimated detectors may prove to be of value in nuclear imaging and probably in clinical applications

Gamma‐ray glows associated with thunderclouds have been observed since the 1980s, however it remains unclear how, and at which thunderstorms gamma‐ray glows are generated in dense atmospheres. In this study, we report the first Compton camera imaging of a gamma‐ray glow from a winter thundercloud. On 14 January 2022, using two identical Bi4Ge3O12 scintillators in energy range of 0.05–5 MeV, we detected two gamma‐ray glows lasting ∼4 min in a mountain area 25 km from the Japan Sea and 410 m above sea level. The same events were also observed by the Compton camera, where the first glow we observed suggested statistically significant (4.0 and 5.9 σ level) signals of two enhanced concentrations in gamma‐ray photon images in a range of 0.15–1.5 MeV. These concentrations were most clearly observed in a time window of Δt = 50 s around the peak intensity of the gamma‐ray glow.
