Multicenter Assessment of Ultrasound of the Spermatic Cord in Children With Acute Scrotum

Abstract: High resolution ultrasonography based management of acute scrotum is reliable and reproducible. Thanks to its high sensitivity and specificity for the diagnosis of spermatic cord torsion, high resolution ultrasonography can significantly improve the management of acute scrotum in children.

“…In a larger multicenter study of 919 patients with an acute scrotum, HRUS was used to detect spermatic cord torsion in 199 of 208 patients with surgically proven testicular torsion (96% sensitivity), compared with 158 confirmed on CDS (sensitivity 76%). 32 Whilst HRUS revealed a linear cord in the remaining 711 patients (99% specificity), the authors did show that increasing reliability correlated with the degree of experience by the radiologist, and that visualization of a spermatic cord twist is difficult in very high or inguinal testes.…”

“…Though color Doppler US is a reliable test in the diagnosis of TT, falsenegative results can be seen. Kalfa et al (19) described the advantages of high-resolution US (HRUS) which enables direct visualization of the rotation of spermatic cord. In a study performed on 919 patients, the sensitivity of color Doppler US was determined as 76%, while sensitivity and specificity of HRUS were 96% and 99%, respectively (3).…”

“…Normal intra-testicular perfusion does not exclude the possibility of torsion and there remains a falsenegative rate in this modality. 51,52 As a missed diagnosis of testicular torsion is one of the most common medicolegal claims in adolescent patients in the United States, 53 we must stress again the prime importance of clinical evaluation and decision. Nuclear scintigraphy and MRI are not popular for the same reason.…”
