Multicenter Prospective Trial of Stent Placement in Patients with Symptomatic High-Grade Intracranial Stenosis

Abstract: BACKGROUND AND PURPOSE:On the basis of the high 1-month stroke and/or death (14.7%) rates associated with stent placement in the Stenting versus Aggressive Medical Management for Preventing Recurrent Stroke in Intracranial Stenosis trial, modifications in patient selection and procedural aspects for intracranial stent placement have been recommended. We performed a multicenter prospective single-arm trial to determine whether such modifications would result in lower rates of periprocedural stroke and/or death.

“…Thus our results are promising but also underline the periprocedural risk of eICS. Previous studies of the post-SAMMPRIS era, mostly investigating eICS for ICAD in Asian populations, reported low rates of death and stroke recurrence on day 30, ranging from 2.8% to 4.4% 10 12–14. However, these promising results might not be applicable to western populations because of the ethnic component of ICAD,risk factors, incidences, and comorbidities of symptomatic intracranial stenosis possibly differ and therefore outcome results might require a different interpretation 15.…”

“…Gao et al enrolled patients with recent TIA or ischemic stroke related to high-grade stenosis of a major intracranial artery and with distal hypoperfusion and/or cortical involvement but excluded patients who had ischemic symptoms within the recent 3 weeks and perforator ischemic events. As a result, the overall 1-month stroke and/or death rate was 2% (2/100) [23]. Characteristics of the above trails were summarized in Table 2.…”

“…Patients with ischemic symptoms related to high-grade intracranial stenosis (70-99%) have an almost 20% risk of recurrent stroke within one year despite antithrombotic treatment [1]. Studies demonstrate that in the setting of maximal medical therapy, a large cohort of patients will remain symptomatic [2].…”
