Multicentric Reticulohistiocytosis

Abstract: A case of multicentric reticulohistiocytosis in a 49-year-old man associated with polymyositis is presented. The clinical features along with the light and electron microscopic studies are commented. Partial improvement of the skin lesions was achieved after treatment with azathioprine but the patient’s general condition remained unchanged.

“…Therapies with corticosteroids, hydroxychloroquine, methotrexate, alkylating agents such as chlorambucil and cyclophosphamide, and azathioprine have been used with mixed success. [1][2][3][4] The challenge in this case was to determine a safe and effective treatment for both the skin and joint manifestations of the disease.…”

“…These findings strongly suggest that the improvement of multicentric reticulohistiocytosis could be accounted for by the alendronate treatment. In the case reports of multicentric reticulohistiocytosis described thus far, more aggressive therapy, such as a combination of corticosteroids with immunosuppressive agents (3, 13–15), has met with some success, although a satisfactory result was not obtained, and sometimes there were severe adverse effects, such as nephrotoxicity, myelosuppression, and lymphocytopenia (3, 4).…”

“…[10][11][12][13] Our case suggests a role for azathioprine in the treatment of the cutaneous and arthritic manifestations of MRH. Further support for this is provided by the disease recurring upon cessation of azathioprine and resolution with its re-introduction.…”
