Multicentric Study Comparing Intravesical Chemotherapy Alone and With Local Microwave Hyperthermia for Prophylaxis of Recurrence of Superficial Transitional Cell Carcinoma

Abstract: In our series, endovesical thermochemotherapy appears to be more effective than standard endovesical chemotherapy as an adjuvant treatment for superficial bladder tumors at 24-month follow-up, despite an increased but acceptable local toxicity.

“…Freedom from tumor recurrence, the primary endpoint was significantly improved with addition of hyperthermia to 83% as compared to 42% with mitomycin C alone. (93) In a subsequent report of long-term results, this benefit was maintained with median follow-up for tumor free patients of 91 months. 10 disease-free survival was 53% vs. 15%…”

“…Although only a small number of these patient have a port-a-cath in the re-irradiation field, a good knowledge on how and to what extent a metal or silicone port-a-cath disturbs the SAR distribution of an applicator is important for a safe and effective superficial hyperthermia treatment. Further, we notice there is a growing discussion [18] towards the combination of hyperthermia with chemotherapy [19][20][21][22][23]. In that patient group, inclusion of a porta-cath in the hyperthermia treatment field might be a more regular occurrence, and hence stresses the need to know how close to the port-a-cath we can heat.…”

“…In a multicentre, prospective, randomised trial, Colombo et al evaluated the efficacy of adjuvant CHT in 83 patients with primary or recurrent NMIBC [21]. Patients were randomised following TURB complete transurethral resection of bladder tumour to chemotherapy alone (MMC, 41 patients) or chemotherapy combined with hyperthermia (MMC þ HT, 42 patients).…”

“…Posterior wall thermal reaction, a phenomenon typically seen on cystoscopy after microwave-induced CHT, is caused by the location of the RF antenna. It appears as a medallion-shaped discoloured scar-like patch, which in general resolves spontaneously and is not associated with symptoms [9][10][11]21,[23][24][25][26][27][28][29][30].…”
