Multimodal ultrasonographic assessment of leiomyosarcoma of the femoral vein in a patient misdiagnosed as having deep vein thrombosis

Abstract: Rationale:Primary leiomyosarcoma (LMS) of the vein is a rare tumor that arises from the smooth muscle cells of the vessel wall and has an extremely poor prognosis. This tumor can occur in vessels such as the inferior vena cava, great saphenous vein, femoral vein, iliac vein, popliteal vein, and renal vein; the inferior vena cava is the most common site. LMS of the femoral vein can result in edema and pain in the lower extremity; therefore, it is not easy to be differentiated from deep vein thrombosis (DVT). Mo… Show more

“…[ 20 ] To date, there is only one report describing CEUS imaging features for the diagnosis of venous LMS. [ 21 ] The CEUS imaging characteristics in our case and literature both demonstrate a rapid, high-level, heterogenous wash-in and wash-out, which is probably caused by tumor heterogenous perfusion or necrosis. In this case, there was a clear margin during the enhancement, and this may correspond to the pseudocapsule of the tumor.…”

“…The first description of venous LMS was presented by Perl in 1871. This malignancy originates from the artery smooth muscle layer [ 11 ]. The most frequent site for LMS is the retroperitoneum, accounting for up to 50% of all cases, 50% of reported vascular LMS originates from the vena cava inferior and 15% from the iliac veins.…”

“…This approach can prolong survival and improve quality of life. Although a multidisciplinary treatment is advised, the effectiveness of adjuvant radiotherapy, chemotherapy, and hormonal therapy is unknown [ 11 ].…”

“…The female and male distribution is equal and it mainly affects patients in their sixth and seventh decade of life (Cangiano et al, 2017). They often present indolent symptoms, such as soft tissue swelling and mild pain, leading to late and complex diagnosis (Zhang et al, 2017). In approximately 10–25% of cases distant metastasis are identified at the time of diagnosis (Bibbo & Schroeder, 2011).…”
