Multinodular leiomyoma of the esophagus with internodular hydropic degeneration and bulbous serosal protrusions similar to cotyledonoid uterine leiomyoma

Abstract: Leiomyoma is the most common benign tumor of the esophagus, and usually occurs as a solitary mass. We report a case of esophageal leiomyoma which shows multinodular growth pattern with bulbous serosal protrusions. The patient was a 26-year-old woman who had an esophageal ulcerated mass near the gastroesophageal junction. Clinical findings were consistent with carcinoma of the gastric cardia. Based on the clinical diagnosis, total gastrectomy with distal esophagectomy was performed. Histologic appearance of the… Show more

“…We are aware of a few examples of leiomyoma with unusual growth patterns arising in the gastrointestinal tract. 1,2 Those tumors were composed of intersecting fascicles of long spindle cells with classic smooth muscle cell cytology, different from those of the cases reported in this letter. Thus, we agree with Takahashi et al 4 that this tumor does not seem to represent any of the wellcharacterized entities, and warrants special recognition.…”

“…We thank Dr Rotimi for his comments regarding our recent publication in this journal. 1 As Dr Rotimi suggested, a control group of patients with reflux esophagitis would be ideal to evaluate whether duplication of the muscularis mucosae (MM) is part of the metaplastic process in Barrett esophagus (BE) or whether it might instead be a manifestation of reflux esophagitis itself. Presumably, the ideal control group would consist of patients who had a history of erosive esophagitis owing to reflux, as MM duplication (if it occurs outside of the BE setting) would most likely arise during the healing phase of mucosal erosions or ulcerations.…”

“…Over the last 10 years in the literature, there has been no report of death after an enucleation of oesophageal leiomyoma; furthermore, complications such as pleural haematoma, postoperative infections or lack of tightness after an enucleation are very rare [8, 11, 15]. However, no differences have been observed from the results of surgical treatment and oncological treatment for rare malignant oesophageal tumours versus squamous cell carcinoma or adenocarcinoma of the oesophagus.…”
