Multiparameter Monitoring with a Wearable Cardioverter Defibrillator

Abstract: A wearable cardioverter-defibrillator (WCD) is a temporary treatment option for patients at high risk for sudden cardiac death (SCD) and for patients who are temporarily not candidates for an implantable cardioverter defibrillator (ICD). In addition, the need for telemedical concepts in the detection and treatment of heart failure (HF) and its arrhythmias is growing. The WCD has evolved from a shock device detecting malignant ventricular arrhythmias (VA) and treating them with shocks to a heart-failure-monitor… Show more

“…Telemedical and non-invasive techniques are highly sought after for monitoring patients and as presented in the introduction, in cases such as melanoma, they are gaining support to replace medical examinations [5]. They are also widely used in monitoring other medical issues and vital signs including heart failure [35], respiratory [36], heart rate variability [37], fall detection [38] and anxiety [39]. These examples do not require complex laboratory equipment or a controlled environment and in most cases can be performed in the comfort of the patient's home [40].…”

“…In instances where there is a transient risk for SCD, individuals may be considered suitable candidates for a wearable cardioverter defibrillator (WCD). The WCD also serves as a viable alternative for individuals awaiting ICD implantation or those following ICD explanation, such as cases related to infections or endocarditis [89].…”

“…All these features and the amount of remotely transmitted data allow a rigorous management of the patient, giving the physician the possibility to identify the need for a therapeutic adaptation thus potentially avoiding unplanned access to the Hospital for acute decompensation events. 81 Potential functions that could be added in the future are the calculation of heart rate variability and the measurement of pulmonary congestion, the latter already included in another device of the same manufacturer.…”
