Multiparametric MRI for Suspected Recurrent Prostate Cancer after HIFU:Is DCE still needed?

Abstract: • MpMRI has the capability to detect PCa recurrence in post-HIFU monitoring. • The sensitivity of T2w and DWI for detecting PCa recurrence was not improved by DCE. • Readers with different degrees of experience did not improve their performance with DCE.

“…These results stand in contrast to the previously published excellent results for mpMRI to detect recurrent disease after HIFU therapy, with sensitivities ranging from 94% to 97% (2,23). However, all previously published studies used transrectal biopsies with or without MRI guiding as a reference standard.…”

“…However, all previously published studies used transrectal biopsies with or without MRI guiding as a reference standard. In the study published by Lotte et al, 24 of 98 patients were excluded because they did not undergo any biopsy, as was probably due to a negative mpMRI result (23). The use of mpMRI to guide biopsy gained wide acceptance and is ideally performed in patients without previous intervention.…”

“…Several other studies have compared a “biparametric reading session” out of a full mp-MRI protocol with the results of the latter in biopsy-naïve men (79) as well as in the setting of tumor recurrence assessment after therapy (15). All showed that bp-MRI and mp-MRI had similar or even higher (16) diagnostic accuracy as the full exam, but that bp-MRI has the advantage of being shorter and cheaper.…”

“…In addition, there is some evidence that DCE sequences may improve planning for focal therapy 22 or staging tumour at the capsule [23][24][25] . Finally, there is a consensus that DCE is useful after radiotherapy or ablation 26,27 , though the size of the effect when high quality DWI is used has been questioned 28,29 . These potential benefits of contrast must be weighed against its expense and potential risk 30 .…”
