ObjectiveThe objective of this study was to test the feasibility and acceptability of a treatment for weight bullying.MethodParticipants who had experienced weight‐related bullying and were currently experiencing traumatic stress were recruited and enrolled in a feasibility trial of trauma‐focused cognitive behavioral therapy combined with cognitive‐behavioral therapy for eating disorders (TF‐CBT‐WB). Thirty adolescents (aged 11–17) were determined eligible and 28 began treatment (12 weeks).ResultsThis study demonstrated the treatment feasibility and acceptability of TF‐CBT‐WB for adolescents with traumatic stress following weight‐bullying experiences. Overall retention and treatment satisfaction were good. Within‐subjects improvements were observed for intrusion symptoms of traumatic stress, global eating‐disorder severity, overvaluation of weight/shape, dissatisfaction with weight/shape, dietary restraint, and depression. Clinically‐meaningful improvements were attained for several patient outcomes. Clinically‐meaningful decreases in functional impairment were attained by more than half of the participants.ConclusionsOverall, this clinical trial testing TF‐CBT‐WB for adolescents experiencing traumatic stress following weight‐bulling experiences demonstrated therapy feasibility, acceptability, and initial evidence that clinically‐meaningful improvements in patient outcomes were feasible. However, some patient outcomes thought to be more central to how the youth viewed the world failed to show improvements, suggesting that additional content related to these constructs might yield greater benefit.Trial RegistrationThis pilot study was registered on clinicaltrials.gov: NCT04587752, Cognitive‐Behavioral Therapy for Weight‐related Bullying.

ObjectiveEating Disorders (EDs) often start in adolescence, though the prevalence, trajectory of symptom onset and predictors of illness are poorly understood, especially across sociodemographically diverse youth. Here, we leverage data from the Adolescent Brain Cognitive Development℠ (ABCD) Study, a large demographically representative longitudinal sample of youth in the US, to characterize the prevalence of parent- and youth-reported ED symptoms and their sociodemographic characteristics at baseline (ages 9-11) and two-years later (2-year; ages 11-14).MethodA tetrachoric factor analysis summarized clusters of ED symptoms, which were compared between parent and youth reports. Cognitive, mental and physical health variables at baseline were used to predict youth-reported symptoms at 2-year using a mixed-effects logistic regression.ResultsThree factors emerged reflecting “weight distress”, “weight control”, and “bingeing”, with prevalence rates ranging from 1.5 to 7.3%. All symptoms loaded on similar factors between reporters. Rates of symptom endorsement were similar for males and females, with disproportionately higher rates across factors for youth who self-identified as sexual minority, Hispanic, Black, or Mixed race participants, and those from a disadvantaged socioeconomic background, compared to the full ABCD sample. Youth and parent reports at 2-year showed ∼17% overlap. A distinct pattern of cognitive variables emerged as significant predictors of later youth-reported ED concerns, whereas mental health symptoms and traits were non-specific and associated with all ED factors.ConclusionIdentifying the landscape of ED symptoms across demographic groups, reporters and their premorbid factors in late childhood is critical to inform prevention and early intervention efforts, with particularly important implications for historically understudied racial and sexual minority groups.

Autistic people and those with attention deficit hyperactivity disorder are at a high risk of developing an eating disorder. While there is limited evidence on the relationship between other forms of neurodivergence and eating disorders, research suggests associations between giftedness, intellectual disability, obsessive–compulsive disorder, psychosis, Tourette’s syndrome, and disordered eating. Factors underlying disordered eating and/or eating disorder risk for neurodivergent people are multifaceted and complex, encompassing a wide range of intertwined psychosocial, environmental, and biological processes. Moreover, research shows that neurodivergent individuals experience poorer treatment outcomes compared to neurotypical individuals. However, there is a paucity of research in this area overall. More specifically, lived experience-led research remains rare, despite its critical role for improving individualised eating disorder care, as well as mental healthcare more broadly. Indeed, the importance of eating disorder care individuation is increasingly being recognised, particularly within the context of neurodivergence, given the heterogeneous experiences and support needs of neurodivergent people affected by disordered eating and/or eating disorders. Furthermore, despite documented overlaps between various forms of neurodivergence (e.g., co-occurring autism and attention deficit hyperactivity disorder), research looking at eating disorders in the context of neurodivergence through a transdiagnostic perspective is scarce. This lived experience-led narrative review aims to shed light on the intersectional factors underlying elevated disordered eating and/or eating disorder risk for neurodivergent individuals. First, an overview of prevalence data is provided, followed by a thematic framework identifying factors underlying disordered eating and/or eating disorder risk in relation to neurodivergence. A critical appraisal of current eating disorder research and care is then offered before suggestions for neurodiversity-affirming eating disorder care are made. In this view, this paper offers a foundation for future empirical work in this nascent field of inquiry by providing a lived experience-led, transdiagnostic, and intersectional account of eating disorders in the context of neurodivergence.
