Multiple organ dysfunction syndrome and death secondary to Cyberlindnera fabianii

Abstract: Cyberlindnera fabianii is a yeast present in soil rarely associated with invasive infection. Due to advanced diagnostic and therapeutic techniques, pathogenicity is increasingly recognized. A 37-year-old male with B cell lymphoma on rituximab developed multiple organ dysfunction syndrome secondary to C. fabianii bacteremia. Specialized species identification techniques were required after failure of standard methods. Despite extracroporeal membrane oxygenation (ECMO… Show more

“…However, recently, few case reports have highlighted its pathogenic activity in high-risk patients, thus recognizing that fungus is an emerging pathogen associated with fungaemia, sepsis, and multiple organ dysfunction syndrome. Interestingly, half of reported cases occurred in pediatric patients, while an outbreak of fungaemia involving preterm neonates has also been reported [ 42 , 43 ]. Although data on antifungal susceptibility of C. fabianii are limited, it appears that fluconazole, itraconazole, and posaconazole are less active compared to voriconazole, 5-flucytosine, and echinocandins [ 44 ].…”

“…It has been described under the genus Hansenula, Pichia and Lindnera [270], and next as Cyberlindnera along with 20 other taxa since the genus Lindnera was a later homonym of an already published genus Lindnera in 1866 [271]. C. fabianii rarely been reported as a human pathogen, but due to advanced diagnostic methods and therapeutic techniques, infection has been increasingly recognised [272][273][274][275][276][277][278][279][280][281][282][283][284][285][286][287][288]. One Flo11-type adhesin that contains two Flo11 domains in the N-terminal region, was found in the Pfam database (Table 3).…”

“…In addition, hospital outbreaks have been also reported in pre-term newborns, suggesting the emergence of nosocomial transmission [ 6 ]. The number of described predisposing factors is scarce, but severe immunosuppression, low birth weight and surgical intervention may contribute to the infection [ 7 ]. It is noteworthy that the usage of indwelling catheters is one of the most important sources of infections, further emphasising the role of biofilm development in Cy.…”
