Multiple separate tumors developing in a single salivary gland is rare in previously untreated patients. Tumors that can be multicentric include Warthin tumor, oncocytoma, basal cell adenoma and acinic cell carcinoma. The incidence of multiple primary unilateral pleomorphic adenomas is extremely rare in patients with no prior history of trauma or surgery. We report two cases of primary multicentric pleomorphic adenoma and review the literature. We also subjected one of our cases to a molecular clonality test. The molecular results suggested that the separate nodules were clonally related, arguing against an independent origin.Keywords Pleomorphic adenoma Á Multifocal Á Parotid gland Materials and MethodsThe computerized files of Clinpath Laboratories were searched for cases diagnosed as primary parotid pleomorphic adenoma or mixed tumor over a 10-year period, 1995-2004. A total of 210 cases were identified. Cases with a solitary tumor nodule were excluded and patients who had prior treatment or trauma were excluded. There was one case of a multifocal primary pleomorphic adenoma. A second case was seen in consultation. In each case hematoxylin and eosin (H&E)-stained slides were reviewed to confirm the diagnosis. Clinical case records were reviewed. A paraffin block was available for clonality studies from the non-referred case.

Salivary gland tumors usually occur as single lesions. To have more than one tumor is unusual. We report a case of an adult male who presented with a mucoepidermoid carcinoma involving the minor salivary glands of the palate at age 57 years, followed by an adenoid cystic carcinoma of the floor of mouth at age 63 years. The patient later succumbed to non-Hodgkin lymphoma at age 72 years. There are 31 acceptable cases of multiple malignant salivary gland neoplasms reported in the world literature. Multiple malignant tumors of the same histologic type are more common than those of different histologic type. Bilateral acinic cell adenocarcinoma was the most frequent combination of multiple salivary gland malignancy, accounting for 14 cases (10 synchronous and four metachronous). All involved the parotid glands bilaterally with the exception of one case that involved parotid and submandibular gland. Polymorphous low-grade adenocarcinoma accounted for three of the four cases of multiple malignant tumors involving minor salivary glands. Individuals with a history of malignancy are at risk for the development of additional malignant tumors and should receive appropriate clinical follow-up.

A 58-year-old male patient with a mass in the left side of the neck for 3 months was admitted to hospital. Physical examination revealed two masses; a mobile, painless, left jugular mass (lymphadenopathy?) and a parapharyngeal mass behind the left tonsilla palatina. Computed tomography examination of the cervical region and pharynx demonstrated two soft tissue masses. Th e fi rst mass was in the left parapharyngeal region and about 3.5x3x3 cm in dimensions. Th is mass was well-circumscribed and showed heterogeneous contrast uptake. Th e second one was 4.5x2.5x2 cm, sharply delineated and homogeneous.
