Multiplug paravalvular leak closure using Amplatzer Vascular Plugs III: A prospective registry

Abstract: TPVLC with simultaneous deployment of multiple AVP III occluders is feasible with high device success rate and no significant periprocedural complications. The clinical benefits of reduction of HF symptoms and hemolysis are evident after 30 days and persist up to 1 year without recurrence of PVL.

“…It was semi-quantitatively defined as a reduction of paravalvular flow by at least one degree and might correspond to our device success. Another paper on the efficacy of multi-plug TPVLC technique using AVP III reported procedural success, demandingly defined as a reduction of regurgitation to not more than mild, of 76.5% for mitral and 100% for aortic PVLs [ 9 ]. Those results were similar to these currently achieved with PLD.…”

“…Transcatheter PVL closure, with proven beneficial effect on HF symptoms and hemolysis [ 4 ], has been recently granted a class IIa recommendation by the AHA/ACC [ 5 ]. Transcatheter PVL closure was initially performed as an “off-label” indication with ASD/VSD/PDA occluders and later with vascular embolization devices [ 6 9 ].…”

“…It was first reported by Hourihan et al in 1992 [9]. Over the subsequent years its safety and feasibility were documented both in children [10] and adults [1114]. Despite the lack of randomized trials, consistent data from case series and registries resulted in the TPVLC class IIa recommendation being granted by the American Heart Association/American College of Cardiology [15].…”

“…We decided to perform paravalvular leak closure first to have more space for plugs. Indeed, the size of the leak made us plan in advance the implantation of multiple plugs, which has shown excellent long‐term results in a recent study . In our case the deployment of multiple plugs was made possible by the use of an arterial–venous loop which was able not only to increase support, but also to let us deploy each plug from a different route (the first one from arterial access, the second one from the transseptal sheath), with a fully satisfactory final result.…”
