Murine Typhus: An Unrecognized Suburban Vectorborne Disease

Abstract: Murine typhus, an acute febrile illness caused by Rickettsia typhi, is distributed worldwide. Mainly transmitted by the fleas of rodents, it is associated with cities and ports where urban rats (Rattus rattus and Rattus norvegicus) are abundant. In the United States, cases are concentrated in suburban areas of Texas and California. Contrary to the classic rat-flea-rat cycle, the most important reservoirs of infection in these areas are opossums and cats. The cat flea, Ctenocephalides felis, has been identified… Show more

“…For R. typhi, fatality is generally less than 1-2%, with a global distribution with its main vector, the flea Xenopsylla cheopis. R. felis, also globally distributed, is primarily associated with Ctenocephalides fleas (Civen and Ngo 2008, Reif and Macaluso 2009, Eisen and Gage 2012.…”

“…Murine typhus presents after an incubation period of approximately 7-14 days with acute fever, severe headache, myalgia, arthralgia, and macular or maculopapular rash that affects the trunk and distributes in a centrifugal fashion (present in approximately 50% of cases) (Civen and Ngo 2008).…”

“…R. typhi is maintained transovarially in fleas where it multiplies in the epithelium of the flea midgut. Fleas are capable of multiple feeding, and thus potentially transmitting the Rickettsia to several hosts (Civen and Ngo, 2008;Mouffok et al, 2011;Traub and Wisseman, 1978).…”
