Background An infection with SARS-CoV-2 can lead to persistent symptoms more than three months after the acute infection and has also an impact on patients’ physical activity behaviour and sleep quality. There is evidence, that inpatient post-COVID rehabilitation can improve physical capacity and mental health impairments, but less is known about the change in physical behaviour and sleep quality. Methods This longitudinal observational study used accelerometery to assess the level of physical activity and sleep quality before and after an inpatient rehabilitation program. The study sample consists of 100 post-COVID patients who acquired COVID-19 in the workplace. Group differences related to sex, age, COVID-19 severity, and pre-existing diseases were also analysed. Results Level of physical activity and sleep quality didn’t increase after rehabilitation. Overall, there is a high extent of inactivity time and poor sleep quality at both measurement points. Regarding group differences, male patients showed a significantly higher inactivity time before rehabilitation, and younger patients (< 55 years) spend significant more time in vigorous physical activity than older patients. Post-COVID patients with pre-existing cardiovascular, respiratory, and metabolic disease show slightly less physical activity than post-COVID patients without these comorbidities. Female patients and younger patients showed better sleep quality in some sleep parameters at both measurement points. However, no differences could be detected related to COVID-19 severity. Conclusions Ongoing strategies should be implemented to address the high amount of inactivity time and the poor sleep quality in post-COVID patients.

Hintergrund: Vielen Ärzt:innen in Deutschland ist die Erkrankung ME/CFS noch unbekannt. Innerhalb des Gesundheitssystems zeigen sich immer wieder erhebliche Defizite im Umgang mit ME/CFS-Erkrankten. Ziel der vorliegenden Studie war es daher u. a. zu ermitteln, welchen Fachrichtungen die im Rahmen der Diagnosestellung von den Betroffenen konsultierten Ärzt:innen angehören und ob sich hieraus Hinweise auf das ärztliche Vorgehen im Rahmen des Diagnoseprozesses ableiten lassen. Methode: Als Teil des APAV-ME/CFS-Surveys wurden hierzu die Antworten von 674 erwachsenen ME/CFS-Erkrankten (> 20 J.; 554 ♀, 120 ♂), bei denen bereits eine ärztliche ME/CFS-Diagnose vorlag, statistisch untersucht. Das Sampling erfolgte durch Selbstaktivierung und über das Schneeballprinzip. Die Daten wurden in erster Linie deskriptiv ausgewertet. Zur Betrachtung möglicher Zusammenhänge wurde eine Varianzanalyse durchgeführt. Ergebnisse: Fast ein Viertel der Proband:innen litt nach eigener Einschätzung seit 6 bis 10 Jahren an ME/CFS. Bei 62 % wurde die Diagnose innerhalb von bis zu 10 Jahren nach Beginn der Erkrankung gestellt. Bei 6,4 % dauerte es 21-40 Jahre. Knapp 75 % der Proband:innen konsultierten im bisherigen Krankheitsverlauf 6 bis 15 verschiedene Ärzt:innen verschiedenster Fachrichtungen, insbesondere aus der Allgemeinmedizin, Neurologie, Inneren Medizin und Psychosomatik/Psychiatrie. Die Diagnose wurde am häufigsten von Vertreter:innen der Allgemeinmedizin und der Immunologie gestellt. Im Durchschnitt nannten die Proband:innen 11 bei ihnen häufig auftretende, meist neuroregulative Symptome. Schlussfolgerungen: Die Ergebnisse legen nahe, dass bei ME/CFS im Rahmen der Diagnosefindung in Deutschland bislang ein an einzelnen ME/CFS-Symptomen orientiertes Überweisungsverhalten zu Fachärzt:innen bzw. ein eher willkürliches Kontaktieren von Ärzt:innen verschiedenster Fachrichtungen vorherrscht. Daher sind Aus- und Fortbildungsmaßnahmen zum Thema ME/CFS in allen Fachdisziplinen sowohl im niedergelassenen und stationären als auch im Reha-Bereich dringend nötig. Background: The disease ME/CFS is unknown to many doctors in Germany. Within the healthcare system, significant deficits in dealing with ME/CFS patients are repeatedly revealed. Hence, the aim of the present study was to determine which disciplines the doctors consulted by those affected belong to and whether information on the medical procedure in the context of the diagnosis process can be derived from this. Method: As part of the APAV-ME/CFS survey, the quantitative responses of 674 adult ME/CFS sufferers (> 20 y.; 554 ♀, 120 ♂) who already had a medical ME/CFS diagnosis were statistically examined. The sampling was done by self-activation and via the snowball principle. The data were primarily evaluated descriptively. An analysis of variance was carried out to consider possible relationships. Results: Almost a quarter of the subjects said they had suffered from ME/CFS for 6 to 10 years. Diagnosis was made within 10 years of disease onset in 62%. For 6.4% it took 21-40 years. 75% of the participants consulted 6 to 15 different doctors from a wide range of disciplines in the course of the disease, with a wide range of disciplines, in particular from general medicine, neurology, internal medicine and psychosomatics/psychiatry. The diagnosis was made in particular by representatives of general medicine and immunology. On average, the test persons named 11 mostly neuroregulatory symptoms. Conclusions: The results suggest that in the context of finding a diagnosis in Germany for ME/CFS, referral behaviour to specialists based on single ME/CFS symptoms or rather arbitrary contacting of doctors from a wide variety of disciplines has prevailed so far. Therefore, training and further education measures on the subject of ME/CFS are urgently needed in all specialist disciplines in the resident, inpatient and rehabilitation sectors.
