Mycobacterium senegalense Osteomyelitis of the Distal Tibia: A Case Report

Abstract: Abstract. Mycobacterium senegalense infection is rare. We present the third documented case of M. senegalense infection and the first to involve the musculoskeletal system. A 55-year old immunocompetent male developed chronic osteomyelitis of the ankle and required antibiotic spacers, an Ilizarov external fixator and multiple antibiotic regimens to eradicate the infection.

“…Although 1 study to date has detailed a report of M. senegalense causing human infection in an otherwise healthy young boy after cleaning a fish tank, this case is the first to our knowledge implicating M. senegalense as the causative agent in a PJI in an immunocompetent host 8 . More recently, M. senegalense was described in a case of chronic osteomyelitis of the tibia by Maupin et al 11 . Not only is the pathogen unique but also the method of infection of the periprosthetic space through potential ingestion is peculiar because most periprosthetic joint infections result from either direct contact with the pathogen (such as during surgery) or hematogenous spread from disseminated acute infection.…”

“…Mycobacterium senegalense, a rapid growing mycobacterium known as the principal pathogen of bovine farcy, was not believed to cause disease in any domestic or nondomestic animal apart from Zebu cattle 4-6 . It was not until 2005 that M. senegalense was first implicated as a human pathogen, with only a handful of cases published to date [7][8][9][10][11] .The patient was informed that data concerning the case would be submitted for publication, and he provided consent.…”

“…It is known to cause bovine farcy in sub-Saharan Africa 4. It has been isolated as a human pathogen to cause CRBSI,4 musculoskeletal infection,5 skin and soft tissue infection,6 as well as surgical site infection7 and implant-associated infection 8. A reported case of infected laceration wound by shattered fish tank glass by this pathogen suggests zoonotic potential 6.…”

“…An immunocompetent 3-year-old North American girl got soft tissue infection of Mycobacterium senegalense after traumatic injury [ 6 ]. A North American 55-year-old immunocompetent male developed chronic osteomyelitis of Mycobacterium senegalense after traumatic ankle fracture [ 7 ]. Some following cases of osteomyelitis related to Mycobacterium farcinogenes were reported.…”
