Mycotic cysts: report of 21 cases including eight pheomycotic cysts from Saudi Arabia*

Abstract: Mycotic cysts are uncommonly encountered lesions that can be easily missed, especially in cases with scant fungal elements, thus requiring special stains to detect the organisms. We reported 21 cases of mycotic cyst, including eight pheomycotic cysts, with emphasis on the histopathologic recognition of this unusual entity.

“…The most common sites of involvement are the subcutaneous tissues of distal limbs, such as the feet and arms. 41 Environmental exposure to the fungus through a breakdown of normal barriers (eg, inoculation after trauma or during surgery) is proposed to be a significant risk factor for dematiaceous fungi infection. The higher proportion of male patients (70.6%) observed in our series also suggests an occupational risk for this disease, although surprisingly none of our 17 patients reported a previous episode of minor trauma with skin injury.…”

“…Terbinafine has also been used successfully, particularly in patients failing azole therapy (7). Surgical excision alone has been successful in a number of cases, even in organ transplant patients (12,194,235,387,418,558,587,646,686,720,764). The Mohs surgical technique, which was developed for removing melanoma, may be a useful surgical approach, as it spares tissue and completely removes the pathological lesion in staged surgeries (86).…”

“…In Exophiala infection, the most common sites of involvement are the subcutaneous tissue of distal limbs, such as the feet and the arms . Exophiala has been associated with a history of local skin injury in the weeks or months before the appearance of the initial lesions.…”
