Myocardial revascularization in infants and children by means of coronary artery proximal patch arterioplasty or bypass grafting: A single-institution experience

Abstract: Provided that the left main coronary artery bifurcation was not involved in the stenotic process, surgical arterioplasty of the main coronary trunks led to good functional and anatomic midterm results. On the other hand, variable indications and poorer preoperative cardiac conditions might have contributed to the disappointing results observed after coronary bypass.

“…In their 25 patient case series, Bergoend et al 10 Unacceptably high operative mortality rates in comparison to that of conventional CABG led to the abandonment of this procedure until 1983, where Hitchcock and colleagues, 3 aided by the use of better myocardial protection techniques, successfully revived the concept of surgical angioplasty as a valuable alterative to CABG. The technique continued to produce favorable results in carefully selected patients, and in the largest study to date, Maureira et al 22 report a series of 91 patients undergoing SPA of the LMCA using an autologous pericardial patch.…”

“…Bonnet et al 11 treated all patients with aspirin postoperatively but did not specify the duration of treatment. However, the majority of studies did not comment on any particular anticoagulation protocol after SPA, and no studies commented on the dose of any anticoagulant therapy administered 10,11,18. Discussion This study summarizes the published literature reporting the use of SPA in a pediatric population, focusing on the indications and outcomes of the technique.…”

“…891319 Mid-term and long-term results of the different revascularisation modalities are, however, limited. 16 This is particularly true for younger patients undergoing coronary revascularisation for a surgical indication associated with CHD (rather than secondary to Kawasaki disease or post-transplant coronary vasculopathy).…”
