Myopathies Associated With Hypothyroidism: A Review Based Upon 13 Cases

Abstract: The clinical and myopathological features of 13 patients with a myopathy occurring in association with hypothyroidism are presented. Seven patients had hypothyroid myopathy, including two with the Hoffmann syndrome and one with the Kocher-Debre-Semelaigne syndrome. Five patients had an inflammatory myopathy and one had polymyalgia rheumatica. Serum CK activity was elevated up to 12-fold in the patients with hypothyroid myopathy and returned to normal after treatment with thyroxine. Pathological changes in thes… Show more

“…In less extreme cases, 30% of muscle biopsies revealed features of vitamin D insufficiency-related myopathies. (12,13) Vitamin D insufficiency leads to poor absorption of dietary calcium and phosphate, thus leading to reduced muscle strength. Interaction between vitamin D and vitamin D receptors promotes protein synthesis and positively influences muscle mass and strength.…”

“…Case 1 offers a striking example of confusion between the two disorders, since her muscle biopsy had been erroneously interpreted in view of the underlying poorly controlled hypothyroidism. Apart from the need to consider alternative myopathies in hypothyroid patients with muscular symptoms [6], it is imperative to emphasize that the common histopathological findings in muscle biopsy necessitate the exclusion of GSD-II in the suspicion of a possible hypothyroid myopathy.…”

“…The clinical onset of hypothyroidism as an overt and isolated myopathy occurs rarely, and few case reports have appeared in the literature [3]. We describe here three patients in whom skeletal muscle disturbances occurred before there was clinical and laboratory evidence of an autoimmune hypothyroidism.…”
