Narrow-band Imaging Digital Flexible Ureteroscopy in Detection of Upper Urinary Tract Transitional-Cell Carcinoma: Initial Experience

Abstract: This is one of the first reports regarding NBI for UUT-TCC. From this study, we recommend this technology as a valuable diagnostic method, because it considerably improves tumor detection rate by 22.7% compared with WL.

“…Over the last decade, flexible URS has gained popularity among the urologic community for its possibility of offering conservative management for low-grade, lowstage UTUC without compromising cancer control [2][3][4][5][6]. Overall, UTUC is quite rare [19,20]; therefore, scientific reports are derived from small series of patients who underwent different treatment modalities [21], thus leaving several aspects deserving comprehensive clarification.…”

“…In this context, flexible ureteroscopy (URS) and laser photoablation represent a valid treatment option for these patients [2][3][4][5][6]. While the conservative endoscopic management of UTUC was initially suggested for imperative cases (thus including chronic kidney failure or solitary kidney) [7][8][9], the promising results obtained in terms of cancer control lead clinicians to offer this approach also to elective cases (patients with normal contralateral kidney) [10][11][12].…”

“…La sensibilité de détection peut être améliorée par la vision obtenue en Narrow Band Imaging (NBI) ou à l'aide du photodiagnostic dynamique utilisant l'acide 5-aminolévulinique (5-ALA), notamment pour les lésions de petite taille et de carcinoma in situ (CIS) [23,24]. néoplasiques émettent alors une fluorescence qui facilite leur détection.…”

“…However, despite recent technical advancement in retrograde flexible ureteroscopy, such as increased flexibility [1,2], digitization [1,3], narrow-band imaging [3], miniaturization of instruments, and modular concepts for ancillary equipment [4], that now permit access to the entire renal pelvicocaliceal system [1,3], improve lesion detection [3], decrease the risk of infection, and make maintenance both safer for the endoscope and less expensive [4], endoscopy is still limited to surface views of the urothelium and its pathologies. Cross-sectional imaging on the other hand reliably provides information on tissue status around the UUT [5,6], but fails to distinguish between different histological wall layers.…”
