Nasal biopsy in the early diagnosis of Wegener's (pathergic) granulomatosis

Abstract: The diagnostic value of the nasal biopsy in the early diagnosis of Wegener's granulomatosis and its value in prognosis were examined in 11 patients with a clinicopathological diagnosis of the disease. The vascular lesions found included microabscess in the vascular walls in 82%, leukocytoclastic capillaritis in 73%, fibrinoid necrosis of blood vessels in 45%, leukocytoclastic endovasculitis in 27%, and palisading granuloma in vascular wall in 9% of cases. The extravascular lesions included palisading granuloma… Show more

“…We hypothesized that in the event apoptotic or necrotic neutrophils are not cleared by macrophages, DCs may be recruited. In WG, granulomas containing large numbers of freshly activated, apoptotic, and necrotic neutrophils are present in upper airways early in the development of vasculitis (31)(32)(33). As neutrophils undergo apoptosis, they express PR3 and MPO on their surface (26,34).…”

“…Suppurative granulomas, which contain central stellate abscesses, may be seen in the lymph nodes of patients with cat-scratch disease (Bartonella species) or in infections caused by Mycobacterium abscessus [63,64]. Finally, although palisading granulomas are characteristically associated with Wegener's granulomatosis, these may also be caused by infectious agents [65,66].…”

“…With disease progression, a necrotizing systemic vasculitis develops and affects small‐ to medium‐sized vessels, i.e. capillaries, venules, arterioles and arteries [2,3] Susceptible capillary beds include those of the lung, causing lung haemorrhage, and glomeruli, causing haematuria, proteinuria and loss of renal function.…”
