BackgroundDeterminants of the duration of exclusive breastfeeding (EBF) differ in effect and magnitude across populations. The present study aimed to identify factors associated with discontinuation of EBF in a municipality in northeastern Brazil, including variables that have received little or no attention in previous literature.MethodsThis cohort study involved 1,344 mother-child pairs selected from maternity hospitals in Feira de Santana, Bahia, Brazil. Subjects were followed up for 6 months through monthly home visits, and discontinuation of EBF was recorded. Possible determinants were tested using Cox’s four-level hierarchical survival model, taking into consideration the temporal proximity of the predisposing factors to interruption of EBF. Median duration of EBF was estimated using Kaplan-Meier’s survival curve.ResultsMedian duration of EBF was 89 days. Out of the 19 variables tested, 9 showed an association with EBF cessation; of these, two had never been evaluated in Brazilian studies, namely, mother partner’s appreciation for breastfeeding (hazard ratio [HR] 0.62; 95% confidence interval [95% CI] 0.48-0.79) and limiting the number of nighttime feeds at the breast (HR 1.58; 95% CI 1.11-2.23). Another two variables that had been previously evaluated, but had never been described as determinants of discontinuation of EBF showed association: presence of cracked nipples (HR 2.54; 95% CI 2.06-3.13) and prenatal care provided by public services (HR 1.34; 95% CI 1.17-1.55). Other variables showing associations with the outcome were: guidance on breastfeeding received at the hospital (HR 0.80; 95% CI 0.68-0.92), birth in a Baby-Friendly Hospital (HR 0.85; 95% CI 0.73-0.99), less than or equal to 8 years of maternal schooling (HR 1.34, 95% CI 1.17-1.53), mother working outside the home (HR 1.73; 95% CI 1.53-1.95), and use of a pacifier (HR 1.40; 95% CI 1.14-1.71).ConclusionsThe study confirmed that the factors associated with EBF duration are multiple, variable, and dependent on the population being evaluated. Characteristics that had never been previously evaluated or described, at least in Brazilian studies, behaved as determinants of EBF in the present study, and thus allow to expand the existing list of factors determining this practice.

RESUMOO objetivo da pesquisa foi identificar o padrão de aleitamento materno exclusivo nos primeiros seis meses de vida de crianças nascidas em um Hospital Amigo da Criança e os fatores que contribuíram para o desmame precoce. Estudo de coorte prospectivo com 261 mães e crianças. Os dados foram avaliados utilizando-se a análise de sobrevivência através da construção da curva de Kaplan--Meier e teste de Log-Rank para a análise univariada. Foi realizada análise multivariada utilizando-se o modelo de Regressão de Cox com riscos proporcionais. Ao longo dos seis meses, o aleitamento materno exclusivo praticado com 30, 90, 120, 150 e 180 dias foi 75%, 52%, 33%, 19% e 5,7%, respectivamente. Na análise multivariada, as variáveis que mostraram risco para o desmame precoce foram a intercorrência mamária hospitalar e, na consulta de retorno, a posição inadequada e a associação das duas anteriores. A Iniciativa Hospital Amigo da Criança favoreceu o aleitamento materno exclusivo. ReSUmOEstudo descritivo de abordagem qualitativa cujo objetivo foi identificar o conhecimento das puérperas sobre aleitamento materno exclusivo. Participaram do estudo treze puérperas internadas no Alojamento Conjunto de uma instituição pública na região de Caxias do Sul-RS. Para a coleta dos dados utilizou-se a entrevista semiestruturada. A análise de dados foi realizada através de análise temática, emergindo três categorias: o conhecimento sobre amamentação exclusiva; o processo de amamentação; e as influências das informações recebidas sobre o processo de amamentação. Mesmo recebendo informações de profissionais de saúde no período pré-natal sobre a amamentação, é possível compreender a necessidade de melhorar a comunicação e o acompanhamento das puérperas por estes profissionais, como uma continuidade no cuidado, no período do puerpério imediato, tardio e também no remoto. descritores: Período Pós-Parto; Aleitamento Materno; Conhecimento; Enfermagem. aBStRaCt This is a descriptive study, with a qualitative approach, aimed to identify the knowledge of puerperal women on exclusive breastfeeding. Data were collected between September-October 2011, through semi-structured interview. Thirteen puerperal women, interned in a rooming unit of a public institution in the city of Caxias do Sul-RS, participated in the study. Data analysis was performed using thematic analysis. From the interpretation of information three categories emerged: the knowledge about exclusive breastfeeding, the breastfeeding process and the influences of received information. Even getting information from health professionals in the prenatal period, it is possible to understand that there is a need to improve communication and monitoring of mothers, as a continuity of professional care in the postpartum period, and also later, in the remote. Tuvo como objetivo identificar el conocimiento de las madres sobre la lactancia materna exclusiva hasta el sexto mes de vida. Participaron en este estudio 13 mujeres internadas en una unidad de pensión de una institución pública en la ciudad de Caxias do Sul-RS. Los datos fueron recogidos entre Septiembre y Octubre de 2011, a través de entrevista semi-estructurada. El análisis de los datos se realizó mediante análisis temática. Con la interpretación de la información surgieron tres categorías: el conocimiento sobre la lactancia materna exclusiva, la lactancia materna y la influencia de las informaciones sobre el proceso de lactancia. Mismo recibiendo informaciones de los profesionales de la salud en el período prenatal es posible entender que hay una necesidad de mejorar la comunicación y el seguimiento de las madres, como una continuidad de los cuidados de estos profesionales, en el periodo del puerperio mediato, tardío y lejano.
