Narcolepsy is a chronic sleep disorder that disrupts the regulation of a person’s sleep–wake cycle, leading to significant challenges in daily functioning. It is characterized by excessive daytime sleepiness, sudden muscle weakness (cataplexy), sleep paralysis, and vivid hypnagogic hallucinations. A literature search was conducted in different databases to identify relevant studies on various aspects of narcolepsy. The main search terms included “narcolepsy”, “excessive daytime sleepiness”, “cataplexy”, and related terms. The search was limited to studies published until May 2023. This literature review aims to provide an overview of narcolepsy, encompassing its causes, diagnosis, treatment options, impact on individuals’ lives, prevalence, and recommendations for future research. The review reveals several important findings regarding narcolepsy: 1. the classification of narcolepsy—type 1 narcolepsy, previously known as narcolepsy with cataplexy, and type 2 narcolepsy, also referred to as narcolepsy without cataplexy; 2. the genetic component of narcolepsy and the complex nature of the disorder, which is characterized by excessive daytime sleepiness, disrupted sleep patterns, and potential impacts on daily life activities and social functioning; and 3. the important implications for clinical practice in the management of narcolepsy. Healthcare professionals should be aware of the different types of narcolepsies and their associated symptoms, as this can aid in accurate diagnosis and treatment planning. The review underscores the need for a multidisciplinary approach to narcolepsy management, involving specialists in sleep medicine, neurology, psychiatry, and psychology. Clinicians should consider the impact of narcolepsy on a person’s daily life, including their ability to work, study, and participate in social activities, and provide appropriate support and interventions. There are several gaps in knowledge regarding narcolepsy. Future research should focus on further elucidating the genetic causes and epigenetic mechanisms of narcolepsy and exploring potential biomarkers for early detection and diagnosis. Long-term studies assessing the effectiveness of different treatment approaches, including pharmacological interventions and behavioral therapies, are needed. Additionally, there is a need for research on strategies to improve the overall well-being and quality of life of individuals living with narcolepsy, including the development of tailored support programs and interventions.

Which Specialty Would You Choose? Understanding Public Preferences for Sleep Disorder Treatment in South Korea

Objectives: We aimed to investigate public perception of medical specialties in South Korea that diagnose and treat different sleep disorders.Methods: We conducted a web-based survey between January and February 2022, as part of the National Sleep Survey of South Korea 2022. A questionnaire was administered to a stratified, multistage sample of 4,000 random individuals aged 20–69 years from the general population. Participants were asked to select all sleep disorders they believed required treatment from a list. Subsequently, they were asked to identify the clinical department they would visit for each disorder.Results: Sleep apnea (83.4%) and snoring (82.4%) were widely perceived as sleep disorders requiring treatment, followed by insomnia (76.1%), sleepwalking (72.0%), narcolepsy (52.4%), bruxism (49.6%), rapid eye movement sleep behavior disorder (43.4%), excessive daytime sleepiness (33.4%), restless legs syndrome (30.1%), and sleep talking (18.5%). Regarding departments, otorhinolaryngology was the preferred specialty for snoring (79.7%) and sleep apnea (49.4%). More than half of the respondents (55.2%) indicated that they would consult psychiatry department, followed by neurology department (28.2%) for insomnia. Neurology department is preferred for restless legs syndrome, rapid eye movement sleep behavior disorder, excessive daytime sleepiness, and narcolepsy. “Unsure” was a common response for more than 10% of the disorders, excluding snoring and insomnia, highlighting the gaps in public awareness regarding sleep disorders.Conclusions: Public perceptions of the appropriate medical specialties for different sleep disorders vary and are often inconsistent with medical guidelines. Public education regarding the roles of different specialties in managing sleep disorders may improve care by guiding patients to the appropriate specialties.

Objectives: Narcolepsy is a chronic neurological disorder characterized by excessive daytime sleepiness and abnormal rapid eye movement sleep, both of which significantly impact daily functioning and quality of life. This study aimed to examine the clinical characteristics, comorbidities, diagnostic delays, and quality of life of Korean patients with narcolepsy through a comprehensive nationwide survey.Methods: An online survey targeting members of the Korea Narcolepsy Association was conducted between July and October 2023. The survey collected demographic information, clinical characteristics, comorbid conditions, diagnostic tools, and quality of life assessments using the World Health Organization Quality of Life-BREF (WHOQOL-BREF).Results: A total of 299 individuals participated in the study, with a mean age of 32.2 years. The average diagnostic delay was approximately 11 years, with initial symptoms appearing at a mean age of 16.6 years. Excessive daytime sleepiness was the most common initial symptom. The most frequent comorbidities included depression, obstructive sleep apnea, and anxiety disorders. WHOQOL-BREF scores indicated a moderately low quality of life across all domains, with patients having narcolepsy type 1 experiencing more severe symptoms and lower quality of life scores than those of patients having narcolepsy type 2. Higher education levels, marital status, and regular exercise were positively associated with better WHOQOL-BREF scores, whereas depression, older age, and poor sleep quality were negatively associated.Conclusions: The findings from this survey underscore the lower quality of life, with notable delays in diagnosis and a high prevalence of comorbid conditions. The results suggest a need for improved awareness, timely diagnosis, and comprehensive management strategies that address both the primary symptoms of narcolepsy and its associated comorbidities.
