National Quality Measure Compliance for Palliative Bone Radiation Among Patients With Metastatic Non–Small Cell Lung Cancer

Abstract: Background: There exists wide practice variability in palliative treatment schedules for bone metastases. In an effort to reduce variation and promote high-quality, cost-conscious care, the National Quality Forum (NQF) endorsed measure 1822 in 2012. This measure recommends the use of 30 Gy in 10 fractions, 24 Gy in 6 fractions, 20 Gy in 5 fractions, or 8 Gy in a single fraction for palliative radiation for bone metastases. We report on longitudinal compliance with this measure. Methods: Using the National Canc… Show more

“…The finding of an association between academic cancer centers and use of shorter courses of RT is consistent with results reported in the literature. 11,12 The reasons for this observation are likely multifactorial and vary by center. These reasons include greater access to larger teams and peer training, onsite didactics, journal access, acquisition of capital, and potentially different financial incentives for clinicians.…”

“…Two previous studies that investigated the appropriate selection of dose schedules in radiotherapy for BoM [ 26 , 27 ] reported comparable and lower compliance rates than ours (i.e. the pooled compliance rate of 99% for the use of fractions ≤10 for painful BoM [BoM-1]).…”

“…Patients with lung cancer commonly present with bone metastases. 58 Multiple lytic bone lesions assuredly suggest metastatic disease, so histologic or cytologic evidence of bone metastases is not needed in this case. The patient had an accessible lesion so that a bronchoscopy with EBUS-TBNA provided a cytologic diagnosis.…”
