Primary hyperparathyroidism (P-HPT) is a common endocrine disorder that occurs as a result of adenomas (80-85%), hyperplasias (10-15%) or carcinomas (o1%) of the parathyroid glands. Molecular genetic analyses of heritable P-HPT syndromes have provided considerable insight into the understanding of sporadic parathyroid tumors and hyperplasias. This review will focus on the criteria for classification of parathyroid proliferative disorders and will highlight our understanding of these lesions at the molecular level. Advances in radiological imaging techniques together with the rapid intraoperative parathyroid hormone assay will be reviewed with respect to current treatment approaches for P-HPT.

BACKGROUND AND PURPOSE: 4D-CT is an emerging technique that uses high-resolution images, multiplanar reformats, and perfusion characteristics to identify abnormal parathyroid glands in patients with hyperparathyroidism. This study evaluates the accuracy of 4D-CT for localization and lateralization of abnormal parathyroid glands in preoperative planning for minimally invasive parathyroidectomy vs bilateral neck exploration at a tertiary referral center.

Parathyroid adenoma is the common cause of primary hyperparathyroidism and usually presents with features of hypercalcaemia. In the present case report, we describe a case of a large adenoma of the parathyroid gland presenting with pathological fractures. A 42 year old Male presented with bony pains and fracture of the left humerus after a trivial trauma. A detailed clinical examination and laboratory studies revealed hypercalcaemia secondary to hyperparathyroidism. The patient underwent targeted parathyroidectomy of the involved gland after appropriate preoperative localization. Histopathology of the resected specimen was consistent with parathyroid adenoma. Conclusion is a common cause of primary hyperparathyroidism is a common endocrine disorder occurring as a result of Parathyroid adenoma in 80-85% cases. Patients usually present with symptoms related to hypercalcaemia. A high index of suspicion, careful preoperative workup, localization of the affected gland(s) and surgical excision ensures a successful cure.
