National trends of endoscopic retrograde cholangiopancreatography utilization and outcomes in decompensated cirrhosis

Abstract: There is an increasing trend in therapeutic ERCP utilization in DC hospitalizations nationally. There is an overall decrease in mortality in DC hospitalizations undergoing ERCP. This improvement in mortality suggests improvement in both procedural technique and peri-procedural care as well as overall decreasing mortality in cirrhosis.

“…Over the past decade, we have found that ERCP utilization has been increasing among LT patients, which is in accordance with a similar nationwide study by Solanki et al [12]. A similar trend was observed in a study of decompensated cirrhosis patients [13]. A study of all patient populations from 1988 to 2002 noted a similar dramatic increase in ERCP utilization as well [14].…”

“…In our analysis, we observed a decreasing trend of ERCP utilization overall in patients with acute pancreatitis from 2003 to 2014. Mehta et al have previously done a similar study, which showed a similar decreased overall trend of diagnostic utilization of ERCP, but the study specifically looked at utilization of ERCP in decompensated cirrhosis where it showed an increasing trend [6]. A recent population-based study by Somashekar et al showed an increased prevalence of acute pancreatitis hospitalization (2002)(2003)(2004)(2005)(2006)(2007)(2008)(2009)(2010)(2011)(2012).…”

“…However, because it is invasive, ERCP still has certain risks, such as pancreatitis, which is the most common complication of ERCP. [ 8 , 9 ] Cirrhosis is a common gastroenterological disease. Cirrhosis patients often have ascites, portal hypertension, gastric fundus esophageal varices, hypoproteinemia, coagulation dysfunction, and other problems.…”
