Granulicatella adiacens abscess: Two rare cases and review

Granulicatella adiacens is a nutritionally variant streptococcus species. These bacteria are rarely isolated in the laboratory due to their fastidious growth requirements. These have been mostly reported from bloodstream infections, infective endocarditis, infections of orbit, nasolacrimal duct and breast implants. Here, we are reporting two cases of subcutaneous abscesses caused by G. adiacens. In first case, it was isolated from abscess around elbow joint and second case was a suprapatellar abscess. We have also reviewed the published data concerning diagnosis and antimicrobial susceptibility pattern of Granulicatella infections and included some Indian cases.

In the last few years, there have been works in literature describing clinical cases of infections with bacteria previously considered to be non-pathogenic. So far, these bacteria have been commonly isolated from the natural environment: water, soil, plants, and isolation from a sample of clinical material has often been considered an impurity. Therefore, the aim of the paper was to draw attention to newly emerging bacterial species, their characteristics, biochemical properties, virulence factors and predisposition to selected forms of infection in groups of patients with specific risk factors. The impact on the more frequent isolation of species of bacteria previously considered to be non-pathogenic has several aspects: reliable diagnostic methods, including correct identification of the species, enabling the differentiation of closely related bacteria, the growing group of immune-compromised patients and with infectious risk factors (such as older age, diabetes, tumours, organ transplantation), more susceptible to infection, also with less pathogenic microorganisms. Greater adaptability of bacteria to new environmental conditions, including the human body, are also worth mentioning. The pathogenic effect of these bacteria, mainly related to the production of biofilms and factors conditioning adhesion to host cells or biomaterials, facilitating colonization and subsequent infection, is also important. The progress in medicine that has taken place in recent years has positively influenced the life expectancy of patients with certain diseases and in many situations, it contributed to an improvement in its quality. Unfortunately, it also made it possible to cause infection with bacteria that were previously considered to be low or non-pathogenic.

“…Other aetiologic agents include nutritionally variant streptococci (NVS) such as Abiotrophia and Granulicatella which account for about 5-6% of streptococcal endocarditis (Padmaja et al 2014;Giuliano et al 2012). Multiple case studies have found that endocarditis caused by Granulicatella sp has been associated with higher mortality rate (17%), higher rate for treatment failure (41%), and as high as 27% requirement for surgical intervention (Adam et al 2015;Vandana et al 2010). It is due to its fastidious Rahimi et al Egyptian Journal of Forensic Sciences (2022) 12:35 nature that it is frequently associated with negative blood culture, yielding in late bacteriological identification and increased antibiotic resistance to beta-lactam and macrolides (Vandana et al 2010).…”

Background Infective endocarditis (IE) is a bacterial infection of the heart valves or endocardium, with complications such as valve perforation, ring abscess, fistula, or damage to the subaortic structures. This case report aims to illustrate an atypical complication of IE which is a pseudoaneurysm depicting a periannular hemorrhage. Case presentation We describe a case of a 19-year-old male youth who presented with fever and cough a few days prior to his demise. There was no known risk factor for IE. The autopsy revealed a bulging anterior surface, upper part of the left ventricle which was soft and slightly fluctuant. Cut section of the heart revealed large vegetations affecting the right and left coronary cusps of the aortic valve. The vegetations at the left coronary cusp were mobile, with necrotic and hollow area underneath, appearing like a cavity and containing blood clots. The course of the cavity was determined to be at the periannular region and contained within the myocardium. These findings were consistent with left ventricular pseudoaneurysm. Culture of the vegetations specimen yielded growth of Granulicatella adiacens sp. anti-streptolysin O titre (ASOT) was 400 IU/mL and reported as positive. Conclusions IE secondary to Granulicatella sp is rare and may result in catastrophic complications. Therefore, this case report is intended to highlight the autopsy findings of the disease as well as to create awareness of its subtle clinical symptoms.
