Natural orifice translumenal endoscopic surgery: critical appraisal of applications in clinical practice

Abstract: Although initial human results seem encouraging, it is important that this research be conducted in a way that minimizes bad publicity and describes both favorable and adverse outcomes. Robust trials must be conducted at established centers with appropriate experience and institutional board ethical review to allow safe introduction of this novel technique, with adequate and expedient reporting of serious adverse events. The limitations of the current tools and methods are discussed, and suggestions are made f… Show more

“…Natural orifice transluminal endoscopic surgery (NOTES) has become a focus of great interest (1)(2)(3)(4). In NOTES, natural orifices such as the mouth, anus, urethra, and vagina are used as ports of entry into the peritoneal cavity through which flexible or rigid endoscopic devices are passed to perform the surgery.…”

“…Patients seem to be early adopters of the technology, perhaps owing to a perception that surgical risk is related to incision size, whereas surgeons prefer to wait for more safety and efficacy outcome data. 32 Years after NOSCAR published the white paper on NOTES in 2005, and although various NOTES procedures have now been tested in humans, 25,26,[33][34][35][36] investigators continue to rely on animal models to better understand and overcome several of the barriers precluding NOTES from wide clinical adoption. In brief, we have shown the feasibility of transcolonic NOTES peritonoscopy and abscess drainage in a canine survival study.…”

“…Apesar do entusiasmo inicial e das promissores benefícios acerca das técnicas NOTES, elas ainda não sinalizaram superioridade em nenhuma indicação clínica ainda sobre as técnicas existentes 1,2 . Estudos clínicos e experimentais ainda demonstram que evolução tecnológica é necessária para expandir as possibilidades deste novo campo, possibilitando segurança e eficiência às tecnicas NOTES.…”
