Abstract: A 76-year-old man with congenitally corrected transposition of great arteries (CCTGA) presented with acute inferior wall myocardial infarction and underwent primary angioplasty. Coronary anatomic variations and challenges are discussed. ( Level of Difficulty: Advanced. )
