Navigating complexity of child abuse through intuition and evidence-based guidelines: a mix-methods study among child and youth healthcare practitioners

Abstract: Background: Dutch child and youth health care (CYHC) practitioners monitor and assess the well-being of children. One of their main concerns is identifying cases of child abuse, which is an arduous and sensitive task. In these contexts, CYHC-practitioners use both evidence-based guidelines aimed at increasing the quality of care through rationalised decision-making, and intuition. These two practices are seen as being at odds with each other, yet empirical research has shown that both are necessary in healthca… Show more

“…They would try to gather more information when they sensed ‘something was off’ or had a ‘gut feeling’. This is consistent with a recent study of healthcare providers in primary care in the Netherlands (Erisman et al, 2020). “…A CAN identification process is highly reliant on health practitioners’ personal judgment and a clear understanding of their roles and responsibilities [ 37 ]. This emphasises the need to improve oral health practitioners’ understanding in detecting CAN cases in early stages, provide the necessary support to children and their families, and report to the child protection agencies to provide safer environments to children and adolescents in need.…”

“…Given the complexity of family violence, it is unlikely that a single guideline will suffice [ 37 ]. Even so, having practical guidance from the regulatory authority can increase oral health practitioners’ confidence to take action.…”

“…To make sense of how, when, and why suspicions of maltreatment arise in general practice, studies have applied theoretical concepts such as intuition [ 21 ] and gut feeling [ 15 , 22 ]. We explore this through the concept of uncertainty which is increasingly recognised as a condition for practicing medicine [ 23 ] and is intrinsic to making choices (on treatment, procedures, medication etc.).…”
