Nebulized dexmedetomidine-lidocaine inhalation as a premedication for flexible bronchoscopy: a randomized trial

Abstract: Background: Dexmedetomidine plus opioid infusion after topical anaesthesia with nebulized lidocaine for cough suppression is a commonly used method for flexible bronchoscopy. Recently, the use of dexmedetomidine as an additive to local anaesthetics has been reported to have several advantages over conventional intravenous administration. However, there are no data regarding the use of nebulized dexmedetomidine-lidocaine for topical anaesthesia as a premedication for flexible bronchoscopy. Therefore, this study… Show more

“…INdexmedetomidine crosses the blood-brain barrier easily, as dexmedetomidine is lipophilic, which, coupled with high vascularity of nasal and laryngotracheal mucosa helps dexmedetomidine bypass first-pass metabolism (9-11). This may well apply to the nebulized route, which augments the surface area coverage with a thin layer of the drug (12). We attempted to investigate Neb-Dex and compare its efficacy with IV-Dex for attenuating LI since literature regarding Neb-Dex is primarily restricted to procedural sedation as premedication or helping allay separation anxiety in pediatric age group (13)(14)(15)(16)(17)(18).…”

“…Partly due to the perfect topical anesthesia, the successful completion rate of the procedure in both groups was higher than previously reported ( Ishiwata et al, 2018 ). Premedication with nebulized Dex-lidocaine inhalation could provide better operating conditions for FB; however, a variety of commonly experienced negative sensations have been recorded, such as choking and inability to swallow, increased rate of positive diagnoses, which led to several re-examinations ( Gu et al, 2019 ).…”
