Neck Rejuvenation Revisited

Abstract: Facial rejuvenation requires appropriate identification of deformity to effect the desired changes. Cervical deformities are classified into clinically useful categories based on careful preoperative analysis. A thorough understanding of the anatomic bases for the deformities allows the surgeon to choose the appropriate treatments to achieve consistent and reliable results.

“…One or more of these procedures may be required in any patient, depending on the characteristics which the patient and surgeon desire to achieve. Youthful characteristics of an attractive submental-cervical region include good skin elasticity, distinct inferior mandibular border, a submental-cervical angle within normal limits, adequate mandibular body length and chin projection and a high and posterior hyoid bone position [20].…”

“…In comparison, submental fullness is characterized by a rounded sternocleidomastoid-submental angle and an indistinct inferior mandibular border. 2,13,14 A series of injections with KYBELLA has proven to be an efficacious, nonsurgical way to rebuild the characteristics of the ideal submental profile.…”

“…Finally, Dedo stressed the need for correction of a retrusive chin and an awareness of the role that hyoid location plays in creating an unfavorable milieu for neck rejuvenation. More recently, McCollough, Rohrich, Matarasso, and others 32,33,34,35 have proposed classification systems that similarly categorize patients based on anatomy and degree of aging to assist in the selection of the appropriate technique for correction of the aging face and neck.…”

“…As noted above, Dedo, Kamer, and others have noted the importance of treating the platysma to achieve rejuvenation of the neck. 31,33,38 Additionally, a wide diastasis of the platysma can lead to ptosis of the underlying structures in the anterior midline neck. Finally, the platysma can be attenuated to varying degrees as it ascends in the neck and this should be kept in mind when performing liposuction.…”
