Necrotic Complications after Nipple‐ and Areola‐Sparing Mastectomy

Abstract: The nipple- and areola-sparing mastectomy, although resulting in a relatively high frequency of necrotic complications, is a valuable surgical option for patients with small, peripheral tumors and for women undergoing prophylactic mastectomy. The procedure seems to be safer for women under 45 years of age.

“…approach ranges between 2% and 67% (9,13,(16)(17)(18)(19)(20)(21)(22). A potential rationale for the wide divergence in the nipple necrosis outcomes stems from the variability in surgical methodology; in some cases, outcomes with both autologous and expander implant-based reconstructions were combined.…”

“…A. L. Komorowski и соавт. [54], проанализировав эти факторы, пришли к выводу, что возраст пациентов моложе 45 лет обусловливает снижение риска разви-тия некротических осложнений. В свою очередь, А. М. Munhoz в своем исследовании [50] не наблюдал связи между возрастом больных и развитием некроза САК.…”

“…In addition, there are two critical points to consider: 21 of the identified studies included high-risk patients with ≥T3 tumors and/or lymph node involvement of ≥4 positive lymph nodes, and did not comment on adjuvant RT (5,20,21,27,33,38,40,47). For advanced TN stages, several recent national guidelines have recommended external beam RT to the whole thoracic wall, with or without irradiation of the lymphatic drainage system (44)(45)(46).…”
