Neighborhood deprivation and warfarin, aspirin and statin prescription — A cohort study of men and women treated for atrial fibrillation in Swedish primary care

Abstract: Background-We aimed to study differences in the prescribing of warfarin, aspirin and statins to patients with atrial fibrillation (AF) in socio-economically diverse neighborhoods. We also aimed to explore the effects of neighborhood deprivation on the relationship between CHADS2 risk score and warfarin prescription.

“…We used neighborhood socioeconomic status as a proxy for lifestyle [38], and we used an area-based system to characterize the neighborhoods in which each individual resided in the present study [35,36]. Neighborhood socioeconomic status has also been shown to be associated with having cardiovascular pharmacotherapy prescribed according to guidelines [41]. The results in the present study were attenuated but still significant in men after adjustments for neighborhood socioeconomic status, and drug and alcohol abuse.…”

“…Prescribed cardiovascular drugs have also been shown to be in agreement with guidelines to a higher degree in high-SES neighbourhoods than in low-SES neighbourhoods 27. Yet, the results were still significant when we adjusted for neighbourhood goods and services including access to pharmacies and healthcare.…”

“…Individuals living in low SES neighbourhoods have a lower awareness of healthy life-style changes than individuals in high SES neighbourhoods (27), and have been shown to have an earlier onset of multi-morbidities (28). Moreover, they receive less optimal pharmacotherapy for their AF (29). Yet, the present results of a higher mortality in men with AF and subsequent depression remained significant after adjustments for neighbourhood SES, indicating that the neighbourhood SES does not explain the excess mortality.…”
