Neonatal perforated Amyand's hernia presenting as an enterocutaneous scrotal fistula

Abstract: Perforation of the vermiform appendix in a septic neonate with an Amyand's hernia resulted in the formation of a scrotal enterocutaneous fistula. In conclusion from this exceptional complication, active parental awareness for any neonatal scrotal swelling is required, and an early operative policy for the neonatal inguinal hernia is significant.

“…21 Fistulae involving the urinary tract also included those which projected to the kidney (two cases), 52,53 ureter (four cases), [54][55][56][57] and one case of urachal cyst. 46 Of the patients with fistulae to the skin, 3,4,8,58-89 three of these were within Amyand (inguinal) hernias, 3,66 of which one patient also had a history of Hirschsprung's disease, 4 and another case projected from a De Gangerot (femoral) hernia. 83 The fistulae to vascular structures comprised 14 to the aorta 10,[90][91][92][93][94][95][96][97][98][99][100][101][102] and five to the iliac artery.…”

“…Indeed, Amyand’s hernias have been reported to present as blatant surgical emergencies such as a perforated appendix in an inguinal hernia presenting as Fournier’s gangrene, but also more subtly such as the giant left-sided Amyand’s hernia with an appendecial abscess without any clinical evidence of incarceration, strangulation, acute scrotum, or occlusion encountered by Mongardini et al [ [9] , [10] ]. The spectrum of Amyand’s presentations is equally diverse in pediatric populations as well, ranging from a neonate reported to have an Amyand’s hernia with a perforated appendix and enterocutaenous scrotal fistula to a six-year old boy whose appendix had been perforated by a metal pin [ [11] , [12] ].…”
