Neuroendocrine Dysregulation in Irritable Bowel Syndrome Patients: A Pilot Study

Abstract: Background/AimsIrritable bowel syndrome (IBS) is a multifactorial disorder, involving dysregulation of brain-gut axis. Our aim was to evaluate the neuroendocrine activity in IBS. MethodsThirty IBS and 30 healthy volunteers were enrolled. Psychological symptoms were evaluated by questionnaires. Urinary 5-hydroxyindoleacetic acid, plasma serotonin (5-hydroxytryptamine, 5-HT), endothelin, and neuropeptide Y (NPY), and plasma and urinary cortisol levels were evaluated. Fourteen IBS subjects underwent microneurogra… Show more

“…The majority of PTSD studies (90%) and MDD studies (89%) used well-known diagnostic assessment tools (i.e., structured clinical interviews based on DSM or ICD criteria; Table 1 and Figure 2). However, studies on chronic stress lacked strict diagnostic definitions (Aschbacher et al, 2014;Irwin et al, 1991Irwin et al, , 1997Jonsson & Hellström, 2000;Stasi et al, 2013Stasi et al, , 2017).…”

“…Moreover, independently from the presence of a full‐blown diagnosis, the clinical observation of IBS patients often has highlighted some “vulnerability psychological features,” often noted by the gastroenterologist themselves. For this purpose, we included in the IBS/Psy group not only subjects with psychiatric diagnosis but also patients with other features such as lifetime psychopharmacological (or psychotherapeutic) treatment, indicating an underlying subthreshold psychiatric distress . It is likely that IBS/Psy patients lacking Axis I diagnosis could have suffered any time from psychological reactions, attenuated or subclinical disorder, isolated soft symptoms, or personality trait leading to psychiatric help‐seeking.…”

“…Functional assessments of the stress response and hypothalamic-pituitary-adrenal (HPA) axis activity have been explored in patients with IBS, with some evidence to suggest elevated salivary cortisol levels at awakening [28], and serum neuropeptide Y (NPY) may be useful [29]. A greater frequency of serotonin transporter (SERT) gene variants has been found in IBS patients with a history of either depression or anxiety, and may play a functional role in digestive symptom pathogenesis [30,31].…”
