Introduction. Discrepancies exist regarding the clinical course and prognostic factors for post-COVID fatigue. Therefore, our aim was to assess the timely course of fatigue and its possible predictors in patients previously hospitalised due to SARS-CoV-2 infection. Material and methods.Patients and employees of the University Hospital in Krakow were assessed with the use of a validated neuropsychological questionnaire. Included were participants aged 18 or more, previously hospitalised due to COVID-19, who completed questionnaires only once > 3 months after the onset of infection. Individuals were retrospectively asked about the presence of eight symptoms of chronic fatigue syndrome at four timepoints: before COVID-19, within 0-4 weeks, 4-12 weeks, and > 12 weeks post-infection.Results. We enrolled 204 patients [40.2% women, median age 58 (46-66) years] evaluated after a median of 187 (156-220) days from the first positive nasal swab test for SARS-CoV-2. The most common comorbidities were hypertension (44.61%), obesity (36.27%), smoking (28.43%), and hypercholesterolemia (21.08%); none of the patients required mechanical ventilation during hospitalisation. Before COVID-19, 43.62% of patients reported at least one symptom of chronic fatigue. Within 4, 4-12, and > 12 weeks after COVID-19, the prevalence of chronic fatigue was 76.96%, 75.49%, and 66.17%, respectively (all p < 0.001). The frequency of chronic fatigue symptoms decreased within > 12 weeks following the onset of infection but did not return to baseline values, except for self-reported lymph node enlargement. In a multivariable linear regression model, the number of fatigue symptoms was predicted by female sex [β 0.25 (0.12; 0.39), p < 0.001 and 0.26 (0.13; 0.39), p < 0.001 for weeks 0-12 and > 12, respectively], and age [for < 4 weeks, β -0.12 (-0.28; -0.01), p = 0.029]. Conclusions.Most patients previously hospitalised due to COVID-19 suffer from fatigue > 12 weeks after infection onset. The presence of fatigue is predicted by female sex and -only for the acute phase -age.

Introduction: The clinical signs of COVID-19 include ageusia and anosmia. Anosmia and ageusia haven’t been evaluated as prognostic factors in any prior studies, though. Therefore, the purpose of this review is to assess the effectiveness of ageusia and anosmia as prognostic indicators in COVID-19 patients. Methods: Literature was collected from various databases systematically using the PRISMA until May 25th,2022. The screening process was performed based on inclusion and exclusion criteria, before being analyzed qualitatively. The risk of bias was assessed using Newcastle-Ottawa Quality Assessment Scale converted by AHRQ. Results: Anosmia and ageusia could be used as the indicator for the good prognostic associated with lower mortality, milder trajectory rate, ICU, and hospital admission risk, and shorter length of stay. Anosmia and ageusia have shown high prevalence to predict a prognosis for the COVID-19 infection. Although COVID-19 prognosis also depends on the other lying conditions, patients with anosmia or ageusia had a lower mortality risk due to the lower body mechanism and cell inflammation mechanism toward the viral load that may not lead to the maladaptive cytokine release in response to infection generally called as a cytokine storm. Conclusion: In COVID-19 patients, anosmia and ageusia have been shown to be indicators of a favorable prognosis due to lower disease severity, mortality, risk of ICU and hospital admission, and shorter duration of stay. Therefore, in order to determine the prognosis, it is important to assess the clinical symptoms of the patients.

Чаще всего COVID-19 проявляется как респираторное заболевание, однако растущий массив клинических данных показывает, что неврологические симптомы и осложнения вносят значительный вклад в клинический спектр заболевания, особенно у пациентов с тяжелым течением заболевания. Воздействие на общественное здоровье отдаленных (или даже пожизненных) последствий может быть намного больше, чем острые проявления инфекции SARS-CoV-2. Широкий спектр неврологических проявлений COVID-19 включает в себя острые нарушения мозгового кровообращения, такие как ишемический инсульт, геморрагический инсульт и церебральный венозный тромбоз, у относительно молодых и ранее здоровых пациентов. COVID-19-ассоциированные острые нарушения мозгового кровообращения связаны с гиперкоагуляционными и протромботическими состояниями. Неврологические симптомы и осложнения COVID-19 не обязательно требуют прямого инфицирования структур центральной нервной системы, и могут возникать вторично по отношению к тяжелой системной реакции в ответ на инфекцию SARS-CoV-2 вне нервной системы. Нейротоксичность инфекции SARS-CoV-2 может быть вторичной по отношению к иммуноопосредованному патогенезу и дисфункции коагуляции. Для определения оптимальных стратегий предотвращения долгосрочных инвалидизирующих последствий COVID-19 и обоснования терапевтического выбора необходимы изучение патофизиологических процессов и клинические испытания. Most commonly, COVID-19 presents as a respiratory disease, but a growing body of clinical evidence shows that neurological symptoms and complications make a significant contribution to the clinical spectrum of the disease. The public health impact of long - term (or even life - long) consequences of the disease may be much greater than the acute manifestations of SARS-CoV-2 infection. The wide range of neurological manifestations of COVID-19 includes acute cerebrovascular events such as ischemic stroke, hemorrhagic stroke, and cerebral venous thrombosis in relatively young and previously healthy patients. COVID-19-associated acute cerebrovascular accidents are related with hypercoagulable and prothrombotic conditions. Neurological symptoms and complications of COVID-19 do not necessarily require direct infection of structures in the central nervous system, but may occur secondary to a severe systemic reaction to SARS-CoV-2 infection outside the nervous system. The neurotoxicity of SARS-CoV-2 infection may be secondary to immune - mediated pathogenesis and coagulation dysfunction. To identify the optimal strategies to prevent the long - term disabling consequences of COVID-19 and substantiate the therapeutic choice, it is necessary to study the pathophysiological processes and conduct clinical trials.
