Neuromusculoskeletal disorders in the neck and upper extremities among drivers of all-terrain vehicles – a case series

Abstract: Background: The purpose of this study was to investigate whether professional drivers of allterrain vehicles (ATVs) with neck pain have a different array of neuromusculoskeletal disorders in the neck and upper extremities than a referent group with neck pain from the general population. It is hypothesized that exposure to shock-type vibration and unfavorable working postures in ATVs have the capacity to cause peripheral nervous lesions.

“…We initially focused on this group because the knee is the most commonly affected joint site ( Jinks et al, 2011 ) and there is evidence that people who do not consult their GP for joint pain are less likely to self-manage their knee pain ( Jinks et al, 2007 ). We identified potential participants from respondents ( N = 567) to a preexisting, separately funded, longitudinal survey of joint pain sufferers aged 50+ in the west Midlands region of the United Kingdom ( Thomas et al, 2004 ). Potential recruits were identified using self-reported information collected in the survey questionnaire.…”

“…In one study, a high prevalence of neuropathies was found among drivers of terrain vehicles (Rehn et al 2004), but for road vehicles the exposure is expected to be lower. Forceful movements of the hands as a risk factor for carpal tunnel syndrome (Bernard 1997) may have played a role for drivers of heavy vehicles in the past.…”

“…a head rotation reduces the EMG amplitude in the upper neck [15]. A head posture divergent from neutral is common as a result of work demands [16], with large variations in time spent in that position assumed depending on the work, driver and vehicle [17]. We have not found any study that has investigated the influence of lateral flexion of the head on postural muscles or kinematic reactions from sideways perturbations.…”
