New aspects in ultrasound-guided compression repair of postcatheterization femoral artery injuries.

Abstract: Background This study was undertaken to expand the understanding of ultrasound-guided compression repair (UGCR) of postcatheterization femoral artery injuries. Methods and Results In a series of 62 patients with pseudoaneurysms (n=53) or arteriovenous (AV) fistulas (n=9), UGCR was performed as a nonsurgical method in the treatment of postcatheterization femoral artery injuries. When the communicating channel could be visualized (43 cases), pressure was focused on it; otherwise (10 cases) the extraluminal cavity… Show more

“…Management options for pseudoaneurysm include conservative measures with or without sonographically guided compression therapy [10,11], surgical repair [12], transarterial coil embolization [13] and percutaneous thrombin or alcohol injection [14]. There is no standard treatment protocol because of the rarity of neck pseudoaneurysm and the adequate treatment should be determined on case-by-case basis.…”

“…Besides surgery, occlusion via ultrasound-guided compression [51] or covered-stent-implantation [52] has also been reported. Case-reports are available describing the successful utilisation of N-butyl-cyanoacrylate in the closure of AVFs [53].…”

“…Application of good external compression after sheath removal, selecting a small-bore introducer and sheath removal at a low activated clotting time (<200 sec) are important factors preventing pseudoaneurysm. Ultrasonography guided compression of the communicating canal of the pseudoaneurysm stops the blood flow and leads to clotting and obliteration with a success rate of 84-93% in patients without anticoagulation (Lange et al 2001, Gabriel et al 2007, Schaub et al 1994. Ultrasound-guided thrombin injection is another effective means with a success rate of 97-100% (Schneider et al 2009, Weinmann et al 2002.…”
