New Pharmacological Approaches for the Treatment of Neurotrophic Keratitis

Abstract: Neurotrophic keratitis (NK) is a rare degenerative condition that is caused by damage to the trigeminal nerve, with partial or complete loss of corneal sensory innervation. The loss of innervation leads to impaired healing of corneal epithelium, which subsequently results in punctate epithelial erosions, persistent epithelial defects, corneal ulcers and corneal perforation. Management of NK is often supportive and aims to promote epithelial healing and prevent progression of disease. Multiple novel pharmacolog… Show more

“…It is also crucial that the cornea is assessed for NK as this could significantly affect the response to therapy and surgical wound healing. 5 If Cochet-Bonnet testing demonstrates a neurotrophic cornea, it is even more crucial that this patient continues PRGF maintenance therapy and uses a therapeutic scleral contact lens to optimize and protect his ocular surface. If NK is present, we would be highly cautious to perform any surgical procedure as the eye may not adequately heal postoperatively.…”

“…Thus, DR-SCLs could be loaded with cenegermin. On the other hand, udonitrectag (MW 353.37 Da) [Figure 9] is a low-molecular-weight synthetic peptido-mimetic of human nerve growth factor [61,66]. DR-SCLs could be loaded with udonitrectag due to its smaller size compared to cenegermin.…”

“…Increased lubrication reduces both the shear forces sustained with blinking and the concentration of proinflammatory cytokines on the ocular surface, thus promoting epithelialization. 2,6 Serum tears, whether autologous or allogeneic, are closer in composition to real tears compared with artifical lubricants, and have been shown to be effective in NK treatment. 7 These eye drops contain nutrients including sugars, proteins and vitamins, which promote epithelial growth and regeneration.…”
