New reconstruction for bone integration of non-vascularized autogenous bone graft with better bony union and revascularisation

Abstract: This case series suggests that a key-in-slot technique allows non-vascularized bone graft to be used in complex large phalangeal bone defects. Due to better bone contact, a sufficient perfusion and revascularisation of the non-vascularized bone graft can be achieved for a quicker and stable bony union. This method appears to be an alternative to amputation in selected cases with a satisfactory soft-tissue envelope.

“…Another advantage of VBRO over VBSO is that while the vertebral body is rotated, the bilateral slits are closed. Bone-to-bone contact in the bilateral gap promotes bone healing, which in theory is expected to aid fusion 31. However, its impact on fusion rate should be reevaluated with a follow-up study.…”

“…In the field of reconstructive surgery, the reconstruction of refractory bone defects, such as segmental bone defects that follow the resection of tumors, is a key problem. 1 , 2 To reconstruct large bone defects that cause functional disturbances, autologous bone grafting has been adopted as a gold standard, where bone material is collected from the patient and transplanted to the treatment site or as free flaps with bone. 3 However, these procedures impose a large burden on patients, because they involve a second surgery, only a limited amount of bone can be collected, and such surgery requires hospitalization for treatment.…”
