New, Useful Criteria for Assessing the Evidence of Infection in Sepsis Research

Abstract: OBJECTIVES: The Sepsis-3 definition states the clinical criteria for sepsis but lacks clear definitions of the underlying infection. To address the lack of applicable definitions of infection for sepsis research, we propose new criteria, termed the Linder-Mellhammar criteria of infection (LMCI). The aim of this study was to validate these new infection criteria. DESIGN: A multicenter cohort study of patients with suspected infection who were admitted to emergency departments or ICUs. Data were collected from… Show more

“…The major strength of this study is the clinical medical record review and sample selection by a population-level database. Another strength of our study is the use of a clinically widely applicable definition of infection . The sepsis definitions have stated criteria for organ dysfunction but not for infection, and with variable definitions of infection, patients will be classified differently .…”

“…We used the newly published Linder-Mellhammar Criteria of Infection (eTable 2 in Supplement 1). 19 Bacterial isolates were identified at the clinical microbiology department (Laboratory Medicine Skåne, Lund, Sweden). Definitions of susceptibility were characterized according to European Committee on Antimicrobial Susceptibility Testing (EUCAST) guidelines.…”

“…Infection was defined according to the Linder-Mellhammar Criteria of Infection (LMCI) system. 13 LMCI is a modified version of the International Sepsis Forum definition of infection. The original definitions were developed for the intensive care unit (ICU), but the LMCI has been adapted to identify infection outside the ICU.…”

“… 45 In addition, all diagnostic criteria for infection are scored over five axes (clinical signs and symptoms, radiological findings, laboratory findings and microbiological findings). 46 For all events, the post-hoc probability of true infection will be categorised using a four-point scale (none, possible, probable and definite infection). 45 Treatment, including antibiotics and source control, is prospectively registered.…”

“…A comprehensive list of diagnostic criteria, as well as an assessment of the interobserver agreement associated with these, has previously been published by our group 45. In addition, all diagnostic criteria for infection are scored over five axes (clinical signs and symptoms, radiological findings, laboratory findings and microbiological findings) 46. For all events, the post-hoc probability of true infection will be categorised using a four-point scale (none, possible, probable and definite infection) 45.…”
