NEXThaler, an innovative dry powder inhaler delivering an extrafine fixed combination of beclometasone and formoterol to treat large and small airways in asthma

Abstract: The data show that the NEXThaler DPI is an efficient device for the management of persistent asthma. The evaluation of the inhalation profiles through the NEXThaler DPI demonstrates that device activation and consistent dose delivery occurs at patient achievable inhalation flow rates, and supports the broad utility of the NEXThaler DPI in patients with asthma. Overall, all the effectiveness, efficiency and satisfaction outcomes demonstrate the NEXThaler DPI is easy to use.

“…This effect is likely to be due to the increased entrainment air velocity caused by the delayed entrainment and dispersion of the formulation when the BAM is present. Indeed, earlier works have demonstrated that the NEXThaler ® device starts emitting the drug when the flow rate of the patient reaches a value of about 35 L/min (Corradi et al, 2014;Pasquali et al, 2015;Mason et al, 2015). The higher flowrate means higher air velocity, thus higher turbulence intensity inside the cyclone chamber.…”

“…This level flow rate causes high turbulence intensity inside the cyclone chamber, resulting in efficient detachment of drug particles from the carriers. The detachment is promoted also by the strong collisions between the particles with high kinetic energy and between the particles and the walls of the device (Corradi et al, 2014). While qualitatively the above reasoning seems to be logical and correct, a systematic quantitative analysis of the effect of the inclusion of BAM is still missing in the literature.…”

“…The NEXThaler device also delivers a so-called 'extrafine beclomethasone and formoterol aerosol' with particles micronized to a mass median aerodynamic diameter (MMAD) <1.5 μm. Chiesi claims to achieve higher lung deposition with better penetration into the small airways with such particles, compared to inhalation powders with a larger MMAD, due to lower oropharyngeal losses [88,89]. The magnesium stearate in the formulation facilitates effective dispersion of such small drug particles.…”

“…Other inhalers, such as NEXThaler, have demonstrated good performance despite the degree of bronchococonstriction and underlying disease. In healthy volunteers, 55% of the labelled dose was delivered, compared with 56% in patients with asthma and 55% in patients with COPD [19]. Breezhaler was studied by Choltorpe and colleagues [20], showing that the medium fine particles fraction was 42.6% of the labelled dose, with a MMAD of 2.8 mm for glycopyrronium.…”
